Zusammenfassung
Die Indikation zur Angiographie beim schwerstverletzten Patienten ist aufgrund ihrer Invasivität streng zu stellen. Haupteinsatzgebiete sind die Gefäßdarstellung bei akut durchblutungsgefährdeten Extremitäten zur Therapieplanung sowie die Angiographie mit Embolisation einer akuten Blutungsquelle. Zwischen 1997 und 2007 wurden 3018 Patienten konsekutiv erfasst, bei denen eine Ganzkörpercomputertomographie (Ganzkörper-CT) durchgeführt wurde. Retrospektiv wurden diejenigen analysiert, die innerhalb der ersten 24 h eine Angiographie erhalten hatten. Bei 27 erfolgte in den ersten 24 h eine transarterielle Katheterangiographie. Hiervon erhielten 15 eine rein diagnostische Gefäßdarstellung der Extremitäten vor operativer Revaskularisation. Bei 12 Patienten wurde innerhalb der ersten 24 h die Blutungsquelle erfolgreich embolisiert. Periinterventionelle Komplikationen traten nicht auf. Trotz ihrer Schnelligkeit, Zuverlässigkeit und Sicherheit wurde die transarterielle Angiographie nur selten angewendet. Die rein diagnostische Angiographie wurde mit der Etablierung der Multislice-CT von der schnelleren und weniger invasiven CT-Angiographie (CTA) weitgehend abgelöst.
Abstract
Angiography for multiply injured patients must have strict indications, either as a diagnostic method for planning therapy in severe injuries of the extremities or for embolisation of an acute bleeding source. Our study included 3,018 consecutive trauma patients who underwent whole-body computed tomography (whole-body-CT) from 1997 to 2007. In a retrospective evaluation, we analysed patients who received angiography in the first 24 h. Twenty-seven multiply injured patients received angiography in the first 24 h; 15 received it only for diagnostic reasons before revascularisation, and 12 had a bleeding source treated with embolisation. In the period studied, no complications occurred in any patient, and all bleeding could be stopped. Overall, the indications for angiography are rare. Nevertheless, the interventional treatment is fast and reliable. Purely diagnostic angiography for multiply injured patients has recently been replaced by CT angiography, which is much faster and is usually available and sufficient.
