Zusammenfassung

Hochdruckinjektionen der Hand sind seltene, aber schwere Verletzungen. Wir berichten über eine Hochdruckverletzung des Zeigefingers durch Hydrauliköl, die aufgrund ihrer banalen Erscheinung durch den Verletzten zunächst ignoriert wurde, was zur Behandlungsverzögerung führte. Die operative Behandlung offenbarte eine ausgedehnte Gewebeschädigung durch das ölige Imprimat. Die sekundäre Defektdeckung des Handrückens erfolgte durch eine Leistenlappenplastik.

Abstract

High-pressure injection injuries to the hand are rare. Because the extent of tissue damage is often underestimated, the amputation rate is high, and functional outcome is poor. We report on a high-pressure stream from hydraulic equipment that caused an innocent-looking pinpoint-size injury to a digit of the right hand. Neglect of this injury delayed referral for surgical intervention. The surgical approach revealed extensive deep tissue damage due to injected hydraulic oil, and aggressive debridement was necessary. Secondary closure of the defect was performed by a groin flap reconstruction.
