Zusammenfassung
Verursacht durch die steigende Zahl von Rasanztraumen nimmt auch die Zahl schwerer Beckenfrakturen zu. Dabei hat der Schweregrad des erlittenen Gesamttraumas unmittelbaren Einfluss auf die Entstehung von Beckenverletzungen. Die Mortalitätsrate schwerwiegender Beckenverletzungen liegt bei 20–35% Die Indikation zur operativen Stabilisierung instabiler Beckenfrakturen richtet sich einerseits nach dem Verletzungstyp, andererseits spielen patienteneigene Faktoren wie Alter, Allgemeinzustand, Begleitverletzungen usw. eine entscheidende Rolle. Die traditionellen Methoden der Versorgung (iIiosakrale, transartikuläre Verschraubung, transsakrale Stabilisierung mittels Gewindestangen, transversale, ilioiliakale Plattenosteosynthese, direkte Osteosynthese der Fraktur) sind nicht belastungsstabil und erlauben keine frühzeitige Mobilisierung. Die trianguläre vertebropelvine Aufhängung, bei welcher die Beckenringfraktur an der gesunden Wirbelsäule abgestützt ist, erlaubt die anatomische Reposition und sichere Retention der Fraktur mit hoher biomechanischer Stabilität und damit die sofortige Vollbelastung. Sie wird im vorliegenden Beitrag detailliert vorgestellt.
Abstract
As a result of the increasing number of speed-related traumas, the number of serious pelvic fractures is also increasing. At the same time, the degree of severity of the overall trauma suffered has a direct effect on the occurrence of pelvic injuries. The mortality rate for severe pelvic injury is 20%--35%. The indication for surgical fixation of unstable pelvic fractures is based on the type of injury; however, patient-specific factors such as age, general condition, and concomitant injuries also play an important role. Traditional treatment methods (iliosacral, transarticular screw connection, transsacral fixation using threaded rods, transversal, ilioiliacal plate osteosynthesis, direct fracture osteosynthesis) do not produce weight-bearing results and thus inhibit early mobilization. The triangular vertebropelvic stabilization, whereby the pelvic ring fracture is supported by healthy spinal column, permits anatomic repositioning and safely maintains the fracture in good biomechanical stability, thus enabling immediate weight-bearing. It will be described in greater details in the present article.
