Mittelfristige Ergebnisse der autologen Chondrozytentransplantation mit und ohne Trägermaterialien

Zusammenfassung

Für die Behandlung vollschichtiger umschriebener Knorpeldefekte steht seit nunmehr 10 Jahren die autologe Chondrozytentransplantation zur Verfügung. In zahlreichen Studien konnte gezeigt werden, dass damit auch bei langen Nachbeobachtungszeiträumen gute Ergebnisse zu erzielen sind. Diese sog. klassische ACT ist jedoch technisch aufwendig und bedeutet einen erheblichen Eingriff in die Gelenkintegrität. Durch die Verwendung eines Trägermaterials konnten diese Nachteile fast vollständig beseitigt werden. In der vorliegenden Studie konnte gezeigt werden, dass durch die Verwendung des Trägermaterials kein nachteiliger Einfluss auf das klinische Ergebnis eintrat. Vielmehr fanden sich vergleichbare Ergebnisse im IKDC-Score wie bei einer Vergleichsgruppe mit klassischer ACT. Der Vergleich postoperativ aufgetretener Komplikationen mit Angaben aus der Literatur zur klassischen ACT ergab ein geringes Komplikationsniveau. Typische Komplikationen der klassischen ACT wie Hypertrophie und Arthrofibrose traten in geringerer Zahl auf.

Abstract

Autologous chondrocyte transplantation (ACT) has now been used for over 10 years for the treatment of multi-layer, circumscribed cartilage defects. It could be demonstrated in many studies that good results could be obtained with this method even in the long-term follow-up period. This so-called classical ACT is, however, technically demanding and involves a substantial intervention in the integrity of the joint. By the use of carrier matrices these disadvantages could be almost completely negated. In the present study it could be demonstrated that carrier matrices did not have any disadvantageous influences on the clinical result. Moreover, the results in the IKDC score were comparative to those obtained in the control group using classical ACT. The comparison of postoperative complications with those describe in the literature for classical ACT, revealed a low level of complications. Typical complications of classical ACT, such as hypertrophy and arthrofibrosis occurred only rarely.
