Zusammenfassung
Aufbauend auf den erfolgreich eingesetzten Maßnahmen zur Behandlung umschriebener Gelenkknorpelschäden werden mögliche Perspektiven zu Tissue Engineering und regenerativer Medizin vorgestellt. Große Fortschritte in der Knorpeldefektheilung wurden durch den Einsatz funktioneller Knorpelzellen erreicht. Durch anpassungsfähige, intelligente Biomaterialien und den gezielten Einsatz von Wachstumsfaktoren in Zellkultur und Trägermaterialien sollen die Ergebnisse und das Indikationsspektrum der Rekonstruktionsverfahren weiter verbessert und erweitert werden. Gentechnologische Ansätze tragen zu verbesserter Diagnostik und zum Verständnis der physiologischen Grundlagen neuer zellbasierter Therapien bei. Künftig werden v. a. auch ethische Überlegungen und gesetzliche Rahmenbedingungen die Methoden und Möglichkeiten der Entwicklung neuer pharmakologischer und medizintechnischer Ansätze entscheidend beeinflussen. Fachgesellschaften, Gesetzgeber und Kostenträger sind aufgefordert, den rasanten Fortschritt in der regenerativen Medizin zu würdigen und entsprechend umzusetzen. Es ist anzunehmen, dass durch Tissue Engineering und In-situ-Regeneration bald weitere Fortschritte in der Behandlung erzielt werden können.
Abstract
Possible perspectives on tissue engineering and regenerative medicine are presented on the basis of the measures successfully applied to the treatment of circumscribed damage to joint cartilage. The use of functional cartilage cells brought about substantial advances in cartilaginous defect healing. It is hoped that adaptable, smart biomaterials and the targeted application of growth factors in cell culture and support materials will allow further improvement and expansion of the results of and the range of indications for reconstruction procedures. Genetic engineering approaches are making a contribution to better diagnosis and to our understanding of the physiological bases of new cell-based therapies. In the future, the methods and possibilities of development of new pharmacological and technical approaches will be decisively influenced by ethical considerations and legal limiting conditions. Specialist professional societies, legisators and insurances are challenged to respect the rapid progress in regenerative medicine and to take account of it. It must be assumed that tissue engineering and in situ regeneration will soon make further advances in treatment possible.
