Zusammenfassung
Es wurde ein intraoperativ montierbares, telemetrisches Lastmesssystem für multidirektional winkelstabile Osteosyntheseplatten entwickelt, bestehend aus einer internen Sensor- und Sende- sowie einer externen Leseeinheit. Die Sensoreinheit mit einliegendem Dehnungsmessstreifen (DMS) wird auf das Implantat geschraubt, die Sendeeinheit unter der Haut in einer Gewebetasche platziert. Sie enthält alle für die telemetrische Übertragung notwendigen elektronischen Komponenten. Über ihr wird ein externes Lesegerät, das die induktive Energieversorgung und Datenauslesung gewährleistet, angebracht. Alle internen Komponenten sind hermetisch gekapselt. Die Montage des Systems wurde an einem humanen Tibiapräparat überprüft. Anschließend wurde die untere Extremität oberhalb des Kniegelenks abgesetzt und kontrollierten mechanischen Untersuchungen unter statischer axialer Last unterzogen. Die nach Osteosynthese durchgeführte Montage des Lastmesssystems auf das Tibiaimplantat sowie die Einbettung der Sendeeinheit in eine Weichteiltasche verliefen problemlos, jedoch muss der Kabelanschluss im Bereich des Sensorgehäuses um 90° gedreht sein, um eine unnötige Weichteilspannung des umliegenden Gewebes zu vermeiden. Die mechanischen Untersuchungen zeigten eine enge Korrelation zwischen eingeleiteter Kraft und detektiertem Messsignal.
Abstract
A telemetric load measuring system was developed, which can be intra-operatively mounted on multidirectional angle-stable locking plates. The system consists of a internal sensor and transceiver unit, which are connected by a catheter tube to an internal strain-gauge connection wire, and a external reader unit. The sensor unit contains a strain-gauge and is screwed onto the osteosynthesis plate. The transceiver includes all electronic components necessary for telemetric data transmission. It is intra-operatively placed in a soft tissue pocket. The external reader unit is placed above this pocket for inductive power supply and data read-out. All internal units are hermetically sealed. The system assembly was tested on a human cadaver tibia. Afterwards, the lower extremity was amputated above the knee joint and mechanically tested under controlled axial load conditions. After the osteosynthesis was performed, installation of the load measuring system onto the tibia implant and placement of the transceiver in a soft tissue pocket could be performed without problems. However, it was obvious that the lead-out of the catheter tubing needs to be rotated by 90° to avoid unnecessarily high tension on the surrounding soft tissue. Mechanical investigations demonstrated a close correlation between induced axial load and the signal detected.
