Zusammenfassung
Zur Optimierung der chirurgischen und therapeutischen Behandlung und Steuerung des Heilungsprozesses von Knochenbrüchen wurden Fixateursysteme als intelligente Implantate entwickelt. Technischer Kern hierbei ist ein in konventionelle Implantate integriertes mikroelektronisches Sensorsystem, das die Belastung an den Implantaten misst und telemetrisch an eine externe Einheit übermittelt. Nach biomechanischen Versuchen sowie dem Verträglichkeits- und Funktionsnachweis am Schaf erfolgte die klinische Anwendung bei 6 Patienten. Es zeigte sich, dass mit intelligenten Fixateur-interne-Systemen einerseits die Heilungssituation gut beurteilt werden kann, da die Implantatbelastung in direkter Beziehung zur mechanischen Festigkeit des heilenden Knochens steht. Andererseits können therapeutische Maßnahmen gezielt der mechanischen Situation angepasst und so z. B. Überbelastungen vermieden werden.
Abstract
Internal fixator systems designed as intelligent implants were developed for the purpose of improving the surgical treatment and subsequent therapies applied for bone fractures, and also monitoring of the healing process, to the highest possible levels. The technical basis is a microelectronic sensor system integrated in conventional implants; this measures implant load and transmits it telemetrically to an external unit. Biomechanical investigations and demonstrations of biocompatibility and function in sheep were followed by clinical application in 6 human patients. It was found that with intelligent internal fixator systems the healing situation can be well evaluated, as implant load is directly related to the mechanical stability of the healing bone. In addition, therapies can be adapted to the individual mechanical situation, making it possible to avoid overloading, for example.
