Zusammenfassung
Posttraumatische Pseudarthrosen der Unterarmknochen entstehen gehäuft nach offener Fraktur oder schwerem Weichteilschaden, durch Versagen der konservativen Therapie, unzureichende innere Stabilisierung, Infektion oder eine nutritiv/medikamentös toxisch gestörte Knochenbiologie. Die morphologische Klassifizierung der Frakturheilungsstörung erfolgt analog der Einteilung nach Weber und Cech. Die Behandlung orientiert sich an der Art der Heilungsstörung. Hypertrophe Pseudarthrosen benötigen nur Stabilisierungsmaßnahmen, atrophe Pseudarthrosen mit geringer Pseudarthrosenbreite profitieren von der hochenergetischen extrakorporalen Stoßwellentherapie, bei größeren Defektzonen ist die operative Pseudarthrosenrevision mit stabiler Plattenosteosynthese und Spongiosaplastik erforderlich. Infektpseudarthrosen müssen erst operativ infektsaniert werden, bevor der Knochenaufbau begonnen werden kann.
Im eigenen Krankengut des Jahres 2005 konnten mit obigen Maßnahmen 85% der Pseudarthrosen (n=27) und 100% der verzögerten Knochenbruchheilungen (n=8) erfolgreich behandelt werden.
Abstract
Post-traumatic non-union of the lower arm occurs frequently after open fracture through failure of conservative therapy, insufficient internal stabilization, infection, and nutritive or medical toxic disturbance to the bone. The causes have been known for decades. Morphologic classification of disturbances in fracture healing follows the system of Weber and Cech. Treatment is orientated to the type of disturbance. Hypertrophic pseudarthroses require only stabilization; atrophic pseudarthroses with limited joint gap profit from high-energy extracorporal pulse therapy; with greater breadth of the defect, operative revision with stable plate fixation and spongeous bone transplantation are required. When caused by infection, the pseudarthrosis must be operatively sterilized before osteogenesis can begin. In our own patients from 2005, 85% of pseudarthroses (n=27) and 100% of delayed fracture healing (n=8) were successfully treated with the above methods.
