Versorgung instabiler proximaler Humerusfrakturen

Zusammenfassung

Die Marknagelung proximaler Humerusfrakturen hat die Behandlung dieser oft sehr komplexen Brüche alter Patienten mit Osteoporose wesentlich verbessert. Voraussetzung für den Erfolg ist die Einhaltung bestimmter operationstaktischer und -technischer Regeln. Auch wenn das Verfahren neu ist, zeigen die ersten Nachuntersuchungen prospektiv angelegter Studien gute bis sehr gute Ergebnisse trotz der Heterogenität der Frakturformen und des Patientenguts. Unsere eigenen Ergebnisse beruhen auf der Nachuntersuchung der ersten 20 Patienten. Mit einem absoluten durchschnittlichen Constant-Score von 72,4 Punkten und einem alters- und geschlechtsadaptierten Score von 98,8% können wir an das Resultat anderer Studien anknüpfen. Die Komplikationsrate muss weiter beobachtet werden. In 5 von 20 Fällen sahen wir partielle avaskuläre Nekrosen des Oberarmkopfs.

Abstract

Intramedullary nailing of proximal fractures of the humerus has improved the outcome of their treatment in elderly patients suffering from osteoporosis, in whom they are often very complicated. To be successful, intramedullar nailing requires a specific sequence in the surgical technique. The initial follow-up observations in prospective studies of this new technique demonstrate good and excellent results despite the wide variety in fracture types and patients. Our own results are based on follow-up of the first 20 patients treated with intramedullary nailing. The mean absolute Constant score was 72.4 points, and the the score adjusted for age and sex was 98.8%, which is similar to those found in other studies. The complication rate cannot be assessed until results after longer follow-up periods are available. In 5 of our 20 cases partial avascular necrosis of the humeral head was observed.
