Zusammenfassung
Die ankylosierende Hüftparaosteoarthropathie stellt ein besonderes Rehabilitationshemmnis für Querschnittgelähmte dar. In der vorliegenden Studie wurden 11 männliche Patienten im Alter von 19,5–67,3 Jahren (Durchschnitt 41,3 Jahre) mit ankylosierender Paraosteoarthropathie Stadium IV nach Brooker bei Para- oder Tetraplegie operativ versorgt. Dabei wurde eine Knochenmassereduktion mit simultaner Hüftalloarthroplastik durchgeführt. Zur Anwendung kamen sowohl zementierte als auch nichtzementierte Pfannenimplantate und stets nichtzementierte Schaftimplantate. Postoperativ wurde bei 7 Patienten eine Radiatio mit 7 Gy, bei allen Patienten eine medikamentöse Reossifikationsprophylaxe mit Indometazin durchgeführt. Ab dem 4. postoperativen Tag wurde mit fraktionierter Motorschienenbehandlung mobilisiert. Die 11 Patienten zeigten eine durchschnittliche Steigerung des Bewegungsausmaßes Streckung/Beugung von präoperativ 17° (10°–45°) auf 88° (0°–120°) nach 3 Monaten. An wesentlichen Komplikationen traten bei 1 Patient die Reluxation nach 8 Wochen mit Zerstörung der Schnapppfanne und bei 1 Patient eine Reossifikation mit Hüftankylose nach 14 Monaten auf. Beide wurden einer Revisionsoperation unterzogen. Insgesamt konnte bei allen 11 Patienten eine wesentliche Besserung der Hüftbeweglichkeit und damit verbunden eine wesentliche Steigerung in der Effektivität des Rehabilitationsprogrammes erreicht werden. Dieses operative Verfahren ist damit eine adäquate und sinnvolle Methode, die funktionelle Rehabilitation zu verbessern.
Abstract
The complete stiffness of the hip joint is a relevant hurdle for consequent and successful rehabilitation of paraplegic patients. From June 1999 to June 2003 11 male patients (mean age 41.3; range 19.5–67.3 years) with paraplegia or tetraplegia suffering from hip ancylosis were operated. All patients were classified in stadium IV of paraosteoarthropathy after Brooker. They were treated with resection of heterotopic bone formation in combination with hip prothesis. We used cemented and uncemented acetabular components and always uncemented shaft prosthesis. Postoperative treatment included an irradiation with 7 Gy in 7 patients and reossification prophylaxis with Indometazin in all patients. With the 4th post-operative day fractionated continuous passive movement started. The patients displayed an increase in hip-extension and -flexion from preoperative mean 17° (Range: 10°–45°) to 88° (0°–120°) after 3 month. In one patient redislocation of the hip prothesis with destruction of the hip pan occurred in week 8, one patient suffered from reossification with hip reancylosis after 14 months. Both patients were successfully reoperated.
In summary all patients showed a significant increase of hip-movement and were therefore able to enhance the effects of their rehabilitation program. We conclude that this method is a useful and adequate therapy in para- and tetraplegic patients.
