Zusammenfassung
Die präoperative Prothesenplanung ist in der Hüftgelenkendoprothetik unverzichtbar. Die Güte der Planung anhand von konventionellen Röntgenbildern ist v. a. von der Standardisierung der Aufnahmetechnik und der Bildqualität abhängig. Computerunterstützte Planungssysteme erlauben zwar die Digitalisierung der Röntgenbilder und die Planung unter Verwendung spezieller CAD-Software, können die Probleme der konventionellen Röntgentechnik aber nicht ausgleichen. Die digitale Röntgentechnik ermöglicht es, die Prothesenplanung ausschließlich am Bildschirm durchzuführen. Mit Einführung der roboterunterstützten Hüftchirurgie wurde durch den präoperativen Einsatz des Spiral-CT unter Verwendung einer entsprechenden Planungssoftware eine dreidimensionale Prothesenplanung standardisiert mit hoher Präzision ermöglicht. In Zukunft ist auch unter wirtschaftlichen Aspekten davon auszugehen, dass mit zunehmender Verbreitung und Optimierung digitaler Röntgentechnik und digitaler Bildverarbeitung unter Einsatz intelligenter Planungssoftware die Hüftprothesenplanung einfach, schnell und zuverlässig am Bildschirm anhand 1 oder 2 Röntgenaufnahmen erfolgt.
Abstract
Preoperative planning is an essential element in successful total hip arthroplasty (THA). When conventional X-ray plates are used for planning a standard technique and high-quality pictures are the main factors determining how good it is. Computer-based systems using digitalized X-ray films and CAD software allow two-dimensional simulation of the operation and some biomechanical calculations. Nevertheless, this technique cannot compensate for the problems associated with X-ray-based planning. The digital X-ray technique makes it possible to plan the replacement wholly on screen. With the introduction of robot-assisted hip surgery, preoperative spiral CT with appropriate planning software has made it possible to carry out 3D planning of hip replacement operations in a standardized manner and with a high degree of precision. We can assume that in the future, as digital X-ray techniques and digital image processing become more widely available and are improved, there will be economic reasons for using intelligent planning software to plan hip replacements simply, rapidly and reliably on screen on the basis of one or two X-ray exposures.
