Zusammenfassung
Die Anzahl der Achillessehnenrupturen ist mit zunehmendem Freizeit- und Breitensport angestiegen. Die häufigste Rissstelle liegt 2–6 cm oberhalb ihres Ansatzes. Die Diagnose kann nach Anamnese, klinischem Befund und ergänzender Sonographie sicher gestellt werden. Die Möglichkeit der sonographischen Darstellung und Kontrolle im Verlauf hat auf die Therapiewahl in den letzten 10 Jahren Einfluss genommen. Bei adaptierbaren Sehnenenden in 20° Plantarflexion kann die Verletzung konservativ zur Ausheilung gebracht werden, und zwar mit Langzeitergebnissen, die denen nach operativer Therapie analog sind. Standardtherapiemethode der frischen Achillessehnenruptur ist die offen chirurgische Naht. Der Anteil konservativ-funktionell behandelter Patienten oder der durch offene Klebung oder perkutane Tenodese Operierten schwankt zwischen 0 und 20%. 50 Patienten wurden nach offener Naht der Achillessehne klinisch, sonographisch und mit dem BIODEX®-Dynamometer nachuntersucht. Die Kraftentwicklung für Plantarflexion der operierten Seite im Vergleich zur Gegenseite betrug im Mittel 96%.
Abstract
Achilles tendon ruptures have become more frequent with increasing leisure and sport activities. The most frequent point of rupture is 2–6 cm above the insertion. This condition can be reliably diagnosed from the history and the clinical findings supplemented by ultrasonography. The availability of ultrasonography for initial imaging and for monitoring of the course of healing has increasingly influenced the choice of therapy in the last 10 years. With coapted tendon ends in 20° plantar flexion, conservative and functional treatment of this injury can yield long-term results comparable to those observed after operative treatment. The standard method of treatment for a fresh Achilles tendon rupture is open surgical suture. The proportion of patients who have undergone conservative-functional treatment or have undergone open surgery with fibrin glueing or percutaneous tenodesis fluctuates between 0 and 20%. After a open suture of a ruptured Achilles tendon, 50 patients underwent clinical and sonographic examination and isokinetic testing with a Biodex dynamometer. On average, the force developed in plantar flexion on the injured side was 96% of that on the uninjured side.
