Zusammenfassung
Die Halsmarklähmung mit Tetraplegie bei Halswirbelsäulenverletzungen stellt eine der dramatischsten Unfallfolgen dar. Gefährdet sind Kinder sowie Patienten mit zervikaler Spinalkanalstenose und verminderter Rigidität der Halswirbelsäule. Die effektive Ruhigstellung der HWS sollte bereits beim Verdacht auf Halsmarkschädigung erfolgen, besondere Vorsicht gilt Bewusstlosen. Die vorsichtige Intubation führt zu keiner weiteren Rückenmarkschädigung, ist aber bei hoher Tetraplegie lebensrettend. Der Verdacht auf eine Halsmarkschädigung rechtfertigt den Hubschraubertransport ins nächstgelegene Rückenmarkverletztenzentrum mit einer Spine Unit. Infolge der Halsmarklähmung kommt es zur neuronal-spinal bedingten Entkopplung mit massiven, vital bedrohlichen Regulationsstörungen, die regelhaft intensivmedizinisch behandelt werden müssen. Die Halsmarkverletzung stellt ein hoch komplexes Krankheitsbild mit einer Wirbelsäulenverletzungen dar. Die Therapie des spinalen Schocks ist vordringlich, die technisch korrekte operative Wirbelsäulenstabilisation ist in der Regel dringlich, bildet allerdings die Basis der weiteren, personell aufwändigen Intensivbehandlung. Über den gesamten, meist 9-monatigen stationären Verlauf drohen Komplikationen, die bei fehlender Sensibilität verkannt und zu einer weiteren Minimierung der Selbstständigkeit führen.
Abstract
Traumatic spinal cord lesion in the cervical spine is one of the most serious sequelae of accidents. Children are particularly at risk of spinal cord injury after accidents, as are elderly patients in view of the decreased rigidity of the cervical spine and progressing stenosis of the spinal canal. At the accident scene even the suspicion of a spinal cord lesion should lead to effective stabilisation of the cervical spine, and special care is needed in the case of unconscious patients. Careful intubation for mechanical ventilation is not dangerous and can be lifesaving in a tetraplegic patient. The suspicion of a cervical cord lesion justifies using an air ambulance to transport the patient to the nearest spinal cord injury unit, since the loss of spinal and central regulation systems and systemic control mechanics mean that medical treatment in a specialized intensive spine care unit is essential. A spinal cord lesion in the cervical section presents a very complex clinical picture, and surgical treatment of the cervical spine is then urgent. The treatment in the intensive care unit is based on technically perfect stabilisation of the spine. Subsequent deficits and typical complications are common in the first 9 months after the injury is sustained and should be watched for carefully, because the absence of pain, motor function and sensibility makes them difficult to recognise.
