Operative Behandlung der distalen Unterschenkelfraktur unter besonderer Berücksichtigung der winkelstabilen Plattenosteosynthese

Zusammenfassung

Distale Unterschenkelfrakturen sind in ihrer Erscheinungsform genauso uneinheitlich wie ihr Entstehungsmechanismus. Es kann sich sowohl um reine distale Schaftfrakturen als auch Schaftfrakturen mit Beteiligung des Sprunggelenks sowie Frakturen des Pilon tibial handeln. Für die definitive operative Stabilisierung stehen Marknagel und Plattenosteosynthese zur Verfügung. Da der Einsatz des Verriegelungsnagels bei sehr kurzen distalen Fragmenten problematisch wird und er für Pilonfrakturen ohnehin nicht zu verwenden ist, bevorzugen wir die Plattenosteosynthese mit winkelstabilen Schrauben zur Frakturversorgung in dieser Region. Im Jahr 2002 wurden von uns18 distale Unterschenkelfrakturen mit winkelstabilen Platten versorgt, 7 davon mit einer neuen seitengerechten Formplatte. Die ersten vorläufigen Ergebnisse werden anhand von repräsentativen Fällen vorgestellt und insbesondere die Komplikationen diskutiert.

Abstract

Distal fractures of the lower leg are just as varied in their clinical picture as in the mechanism of their causation. They can be distal shaft fractures in isolation, shaft fractures with involvement of the ankle joint, or fractures of the tibial pilon. Definitive operative stabilization can be achieved through nailing and through plate osteosynthesis. Since the use of interlocking nail is problematic with very short distal fragments and these nails cannot in any case be used for fractures of the tibial pilon, we prefer plate osteosynthesis with fixed-angle screws for the treatment of fractures in this area. In 2002, we applied fixed-angle plates in the treatment of 18 distal fractures of the tibia, using a new plate formed differently for the left and right legs in 7 cases. The first preliminary results are presented with reference to several representative cases, and in particular the complications are discussed.
