Zusammenfassung
Die vermeintlich "einfache Knöchelfraktur" stellt hohe Anforderungen an Diagnostik und Therapie. Bereits bei der Anamneseerhebung sollte ein kompliziertes Verletzungsmuster erkannt werden. Die Standardröntgendiagnostik muss beim Verdacht auf komplexe Knöchelverletzungen erweitert werden (CT), um das gesamte Verletzungsausmaß erfassen zu können. Die Frakturversorgung erfolgt überwiegend operativ und sollte nach einem standardisierten Algorithmus durchgeführt werden. Fragmentfehlstellungen müssen beseitigt bzw. vermieden oder umgehend korrigiert werden. Sekundäre Korrekturen sind technisch häufig sehr schwierig und oft nur von begrenztem Erfolg. Besonders anspruchsvoll ist die Frakturversorgung bei alten Menschen und Patienten, welche eine Teilbelastung oder vollständige Entlastung des Beins nicht gewährleisten können, aber wegen schlechter Knochenqualität oder aufgrund spezifischer Frakturmerkmale nicht voll belasten dürfen. Neben außergewöhnlichen Operationsmethoden zur individuellen Behandlung ist oft eine spezielle Orthesenversorgung erforderlich. Mit dem Entschluss zur operativen Therapie muss also auch immer ein Konzept zur Nachbehandlung vorliegen.
Abstract
The supposedly "simple" ankle fracture is very demanding in terms of diagnosis and therapy. A complicated injury pattern should be recognized even at the stage of history taking. If there is any suspicion of complex ankle fractures the standard diagnostic X-ray examinations must be supplemented (by CT) to allow detection of the entire extent of the injury. Treatment for the fracture is mostly surgical and should be carried out according to a standardized algorithm. The displacement of fragments must be eliminated or avoided, or if present, corrected at once. Secondary corrections are frequently very difficult technically, and the success obtained is often limited. Fracture care is particularly demanding in old people and in patients who cannot ensure a partial weightbearing or complete relief of pressure on the leg but in whom full weightbearing is not possible because of poor bone quality or specific properties of the individual fracture. Unconventional methods of operation are often needed for individual treatment, as is the provision of a specialized orthesis. When surgical therapy is decided upon a concept for after-treatment must always be prepared before the operation is performed.
