Zusammenfassung
Ziel der Behandlung von Frakturen des oberen Sprunggelenks ist die präzise Wiederherstellung der Gelenkmechanik. Exakte anatomische Reposition und stabile Retention der knöchernen Verletzungen gelten dabei als Eckpfeiler der operativen Behandlung. Seit den Arbeiten von Lauge-Hansen verfügen wir über ein umfassenderes Verständnis des Gesamtausmaßes dieser Verletzung. Diese einfache und sinnvolle Klassifikation findet allerdings bei der präoperativen Therapieplanung nur selten Berücksichtigung. Auch wurden, seit Beginn der Ära von Kernspin- und Computertomographie, nur von wenigen Anstrengungen unternommen, die Ergebnisse der empirischen Arbeiten von Lauge-Hansen auch wissenschaftlich zu überprüfen. Ebenso verbleiben viele Zusammenhänge zwischen Fraktur- und Band(fehl)heilung sowie Arthrose bis dato unbekannt und bedürfen einer systematischen Analyse.
Abstract
In the case of ankle fractures, the object of treatment is precise restoration of the biomechanics of the joint. Accurate anatomical repositioning and stable fixation of the bony injuries are regarded as the cornerstones of the operative treatment. Since the work published by Lauge-Hansen we have had a more comprehensive understanding of the total extent of this injury. It must be confessed that this simple and practical classification system is seldom referred to when treatment is being planned during the preoperative phase. Since the advent of core spin and computed tomography, few workers have invested any effort in scientific testing of the results of Lauge-Hansen's empirical work. In addition, the relationships between arthrosis and bony (mal)union and (defective) healing of ligaments following fractures remain unknown to date and must be systematically analyzed.
