Zusammenfassung
Olekranonfrakturen sind häufige Verletzungen vorwiegend des Erwachsenen. Die Frakturklassifikation sollte neben Lokalisation, Frakturverlauf und Zahl der Fragmente klassifikationsbezogene Therapiekonzepte ermöglichen. Die Zuggurtungsosteosynthese nach Weber und ihre Modifikationen haben sich bei stabiler gelenknaher Frakturzone bewährt. Die Stabilität der Osteosynthese kann bei bestimmten Frakturtypen durch eine oder mehrere ergänzende Zugschrauben erhöht werden. Bei langen Schrägfrakturen oder Trümmerfrakturen sind Plattenosteosynthesen signifikant stabiler und weisen eine geringere Komplikationsrate auf. Knöcherne Defekte erfordern eine Spongiosaplastik. Zur Vermeidung einer funktionellen Einschränkung des Ellenbogengelenks ist die erreichte Stabilität im Rahmen der postoperativen Mobilisierung entsprechend funktionell zu nutzen. Bei korrekter Verfahrenswahl und Nachbehandlung ist die Prognose gut, selbst Frakturen mit Weichteilschaden oder Trümmerfrakturen heilen häufig unter nur geringem Streckdefizit aus.
Abstract
Fractures of the olecranon are common injuries of the adult. Besides location, type of fracture, and number of fragments, the classification of olecranon fractures should include therapy concepts. Tension band wiring according to Weber or other technical variations are common operative treatments in stable fractures, which are localized close to the joint. One or more additional lag screws may improve the stability in special fracture types. In oblique and comminuted fractures, osteosynthesis with plates shows significantly more stability and lower complication rates. Autologous cancellous bone graft is necessary in bone defects. In order to avoid functional limitation of the elbow joint, it is important to use the operative stability achieved for functional treatment in the early postoperative mobilization period. Given the right choice of operative treatment and postoperative mobilization, the prognosis of olecranon fractures is good. Fractures combined with soft tissue damage or comminuted fractures may lead to functional loss of extension in the elbow.
