Korrekturoperationen nach fehlverheilten supra- und diakondylären Oberarmfrakturen

Zusammenfassung

Die Indikation zur Korrektur nach fehlverheilten Frakturen des distalen Humerus ist je nach Höhe der Fehlstellung unterschiedlich. Deformitäten der suprakondylären Region können ästhetisch störend sein, während bei intraartikulären Fehlstellungen eine Vergesellschaftung mit funktionellen Defiziten und Schmerzen möglich ist. Bei funktionellen Defiziten ist eine Arthrolyse des Ellenbogengelenks unverzichtbar. Bei den suprakondylären Fehlstellungen werden am häufigsten die subtraktiven Korrekturosteotomien angewendet. Bei den intraartikulären Deformitäten steht die aufwändige Gelenkrekonstruktion im Vordergrund. Im Folgenden werden die Grundsätze der Behandlung dargestellt. Auch die Nachbehandlung wird diskutiert.

Abstract

The procedure indicated for correction of malunion following fractures of the distal humerus varies according to the level of the malunion. Deformities in the supracondylar region can be an aesthetic problem, while intra-articular malunion can be associated with functional deficits and pain. Arthrolysis of the elbow joint is essential when there is a functional deficit. In the case of supracondylar malunion, subtractive corrective osteotomy is the procedure most often used. Complex joint reconstruction predominates in the case of intra-articular deformity. The principles of treatment are presented in this paper; follow-up treatment is also discussed.
