Zusammenfassung
Die Bedeutung der Fibulaosteosynthese bei Frakturen am oberen Sprunggelenk oder am Pilon tibiale ist unbestritten.Bei sehr weit distal gelegenen Schaftfrakturen mit Fehlstellung können die Funktion des OSG ebenfalls bedroht und deshalb die korrekte Einstellung der Fibula und die operative Stabilisierung notwendig sein.Der Einfluss der intakten oder operativ stabilisierten Fibula auf die Stabilität des Unterschenkelskeletts bzw. der durch Osteosynthese versorgten Tibia wird dagegen sehr unterschiedlich bewertet. Experimentelle Untersuchungen differieren –je nach Ansatz und Durchführung– im Ergebnis ebenfalls stark. Eigene experimentelle Untersuchungen und klinische Erfahrungen sprechen gegen eine grundsätzliche additive Fibulaosteosynthese.Bei gestörter Heilung an der Tibia ist mitunter sogar eine Fibularesektion von Vorteil. Alle Maßnahmen an der Fibula bei der Therapie von Unterschenkelschaft- oder Schienbeinfrakturen unterliegen einer strengen individuellen Indikation!
Abstract
The importance of osteosynthesis of the fibula in malleolar fractures is undisputed. In lesions located very far distally with dislocation the function of the ankle joint can also be threatened.Correct reduction and operative stabilization of the fibula are therefore essential. In contrast, very different values are placed on the influence of the intact or surgically stabilized fibula on the stability of the lower leg skeleton or of a tibia that has been stabilized by osteosynthesis after a fracture.Depending on approach and implementation, experimental investigations have shown very different results. Our own experimental investigations and our clinical experience do not support the idea of performing additive osteosynthesis of the fibula routinely. In some cases of disturbed consolidation, delayed union, or nonunion of the tibia it seems that resection of the fibula can even give a favourable result. In the context of treatment of lower leg or tibia shaft fractures the indications for anything to be done to the fibula must always be considered on a strictly individual basis.
