Zusammenfassung
Die unaufgebohrte Marknagelung ist eine exzellente Methode zur Stabilisierung von Unterschenkelfrakturen.Im Bereich der distalen 2/5 der Tibia steigt das Risiko einer Achsfehlstellung in der koronaren bzw. sagittalen Ebene aber deutlich an. Eine Ergänzungstechnik für ein minimalinvasives, präzises Einlenken des Nagels kann die achsgerechte Osteosynthese erleichtern.Bei der Palisadentechnik werden Kirschner-Drähte perkutan unter BV-Kontrolle eingebracht, sodass der Nagel mit der schrägen Spitze abgleitet und im Markraum zentriert wird.Das resultierende Drehmoment führt zur Ausrichtung von anatomischer Achse und Nagelachse. Seit Februar 2000 wurde diese Technik bei 12 Patienten eingesetzt (m/w: 7/5, Durchschnittsalter 46 Jahre), 7-mal bei frischen distalen Frakturen, 3-mal zur Nagelpositionskorrektur und 2-mal bei Korrekturosteotomien. Alle frischen Frakturen heilten ohne Achsfehlstellung.Bei den Korrekturfällen konnte die Fehlstellung deutlich reduziert werden (koronar von 9,6° auf 1,4° , sagittal von 5,6° auf 3,0°). Diese Technik erlaubt die präzise, achsengerechte Positionierung des UTN in der distalen Tibia sowie die Korrektur fehlgestellter Nagelosteosynthesen und minimalinvasive Korrekturosteotomien in dieser problematischen Region.
Abstract
Unreamed nailing is an established method of achieving internal fixation of tibial shaft fractures.The interlocking technique allows stabilization of distal metaphyseal fractures, but the risk of malalignment in the coronal and sagittal planes increases considerably when the fracture is in the distal two-fifths of the tibia.This study was performed to test the hypothesis that an additional technique for minimally invasive guidance of the nail might make more precise positioning of the nail easier. K-wires are inserted percutaneously so that the eccentric nail slides with the oblique tip along the K-wire and is centred in the medullary canal.The resulting torque aligns the anatomical axis of the distal fragment with the axis of the nail (“palisade method”).Twelve patients (mean age 46) have been operated on using the palisade method since February 2000. In 7 cases acute fractures were fixed, while in the other 5 the method was used in corrective surgery of malaligned distal tibial fractures.There was no secondary loss of reduction or bolt breakage before bony union. All acute fractures healed with anatomical alignment in the coronal and sagittal planes. In corrective surgery, malalignments were reduced from 9.6° to 1.4° in the coronal plane and from 5.6° to 3.0° in the sagittal plane.One patient died after 7 weeks, one diabetic patient with grade III peripheral artherosclerosis underwent a change to external fixation owing to a deep ulcer around the incision for the furthest distal interlocking bolt.The palisade method presented here allows precise positioning of the nail in short distal fragments by means of K-wires introduced percutaneously and reliably prevents malalignment. It is useful in fixation of acute fractures, secondary adjustment of a malpositioned nail and corrective osteotomy.
