Zusammenfassung
Der distale tibiofibulare Syndesmosenkomplex mit seinen 5 definierten Bandstrukturen – Lig. tibiofibulare anterius und posterius, Lig. tibiofibulare interosseum, dem distalen Anteil der Membrana interossea und dem Lig. tibiofibulare transversale – spielt zusammen mit der Membrana interossea und dem distalen Fibulaende eine entscheidende Rolle für die anatomisch-funktionelle Integrität der Malleolengabel.Verbleibende Verwerfungen im distalen tibiofibularen Gelenk sind klassische Inkongruenzarthrosemodelle und dürfen nach Osteosynthesen am oberen Sprunggelenk ebenso wenig wie verbleibende Instabilitäten,Verkürzungen oder Fehlrotationen toleriert werden.Die Syndesmosenstellschraube soll diese Faktoren bei der Entstehung einer posttraumatischen Arthrose verhindern, ist bei unkorrekter Anwendung aber selbst allzu oft Ursache von fixierten Fehlstellungen,Verknöcherungen und andere Komplikationen.Aus diesem Grund müssen die Indikation für eine Stellschraube eng gefasst und die mit der Schraube fixierte Position der distalen Fibula kontrolliert korrekt sein. Diese Voraussetzungen verlangen die Kenntnis der Klassifikation nach Lauge-Hansen und im Zweifelsfall ein postoperatives CT nach allen Stellschraubenosteosynthesen.
Abstract
The tibiofibular syndesmosis, with its five defined ligamentous structures – the anterior and posterior tibiofibular ligaments, the interosseous tibiofibular ligament, the distal part of the interosseous membrane of the leg and the superior extensor retinaculum of the foot – together with the interosseous membrane of the leg and the distal end of the fibula has an important role in the anatomic-functional integrity of the malleolar sulcus of the fibula.Persisting rejection in the distal tibiofibular joint is a classic scenario predisposing to incongruence arthrosis, as are persisting instability, shortening or malrotation.The syndesmosis-adjusting screw is intended to prevent all these situations, which can be factors in the development of a posttraumatic arthrosis, although when wrongly applied it can itself lead to fixed malposition, ossifications and other complications.For this reason a very narrow range of indications for use of an adjusting screw must be laid down and strictly observed, and the position the distal fibula is fixed in by the screw must be correctly monitored. All this means that the operator must be completely familiar with the Lauge-Hansen classification and that a postoperative CT scan is essential after every adjusting screw osteosynthesis.
