Zusammenfassung
Bei der Therapie von Infekt-/Defekt-Pseudarthrosen sind neben Knocheninfektion, Instabilität und Knochendefekt häufig auch Weichteildefekte,Durchblutungs- und/oder Nervenschäden,Begleiterkrankungen und insbesondere Inaktivitätsschäden gleichzeitig zu behandeln. Die komplexe Problematik ist präoperativ individuell genau abzuklären, dann ein individueller Therapiestufenplan zu erstellen und das Vorgehen zwischen Unfall-, Gefäß-,Neurochirurgen,Neurologen und plastischem Chirurg abzusprechen.Unfallchirurgisch sind fast immer 4 Schritte hintereinander auszuführen. Im ersten operativen Schritt wird die Infektion beruhigt,wobei radikale Sequestrektomie, Stabilisierung im Fixateur und Keimvernichtung durch systemische und lokale Antibiose erfolgen müssen. Anschließend erfolgt der frühest mögliche stabile Weichteildefektverschluss und erst danach der Knochendefektaufbau, der heute bei kleineren Defekten mit autologer/autogener Spongiosa oder in ausgewählten Fällen mit mikrovaskulärem Knochenspan, bei größeren Defekten mit Segmenttransport erreicht wird.Parallel zum operativen Programm ist stets ein umfassendes Rehabilitationsprogramm erforderlich.Am Ende der Behandlung ist auch die berufliche und soziale Wiedereingliederung zu bahnen und zu überwachen.
Abstract
Besides addressing bone infection, instability und bone defect the management of infected gap nonunions often requires simultaneous treatment of soft tissue defects, vascular and nervous lesions, concomitant diseases and especially damage resulting from inactivity. In each individual case the complex problems must be evaluated exactly in order to set up an individual treatment plan. Not seldom therapeutic action has to be coordinated between trauma surgeon, vascular surgeon, neurologist and plastic surgeon.As far as trauma surgery is concerned almost always four therapeutic steps have to be carried out in succession. In the first operative step the infection is quieted with radical sequestrectomy, stabilisation by fixator und eradication of germs by systemic and local antibiotic treatment.As early as possible soft tissue defects have to be closed. Only then the reconstruction of the bony defect is carried out.Nowadays, small defects are filled with autologous/autogenous spongiosa, or in selected cases with microvascular bone chips, while larger defects are bridged by segment transport.Parallel an extensive rehabilitation program is always necessary. At the end of treatment the trauma surgeon also has to put vocational and social rehabilitation on the right way and control its progress.
