Zusammenfassung
Dislozierte 3- und 4-Fragment-Frakturen des Humeruskopfs zeigen nach Osteosynthese häufig ein schlechtes funktionelles Ergebnis. Die primäre prothetische Versorgung kann die prätraumatische Anatomie wiederherstellen, sodass eine frühe funktionelle Behandlung der Schulter möglich ist.Gerade bei älteren Patienten brauchen wir eine sichere Möglichkeit der Versorgung, die eine frühe Mobilisation und Entlassung nach Hause gewährleisten. Noch ist jedoch das funktionelle Outcome nach prothetischem Ersatz der Schulter nicht befriedigend,und zwar weitgehend unabhängig vom gewählten Prothesentyp. Jedoch kann durch den primären prothetischen Ersatz durch eine “einzeitige” Versorgung ein stabiles, schmerzfreies Gelenk ohne die Gefahr von Revisionseingriffen aufgrund von Implantatversagen, Pseudarthrosenbildung oder Oberarmkopfnekrose geschaffen werden.
Abstract
Displaced three- and four-fragment fractures of the humeral head involve a high risk of a poor functional outcome after internal fixation. Primary prosthetic replacement can restore the pretraumatic anatomy, and early functional treatment is then possible.Particularly in older patients a safe treatment permitting early mobilization and early discharge home is needed.The functional outcome of hemiarthroplasty of the shoulder joint following trauma is still unsatisfactory, whatever the type of implant used.Nonetheless, a stable, pain-free joint is achieved with a single surgical intervention, without the risk of revision surgery for implant loosening, pseudarthrosis or ischemic necrosis of the humeral head.
