Posttraumatische Osteitis des Femurschafts

Zusammenfassung

Die Osteitis des Femurschafts bedarf aufgrund der raschen Entwicklung einer Markraumphlegmone einer zügigen diagnostischen Erfassung und therapeutischen Vorgehensweise. Das heutige serielle Revisionskonzept in Verbindung mit dem breiten Spektrum der Stabilisierungsträger, der lokalen und intramedullären antiseptischen sowie antimikrobiellen Maßnahmen, flankiert von systemischer Antibiose und spezieller Weichteilkonditionierung, hat in den letzten Jahrzehnten zu einer signifikanten Reduzierung des Übergangs zur chronischen Form und den gefürchteten Folgekomplikationen bis hin zum Fistelkarzinom geführt. Die hinter jedem Fall stehende singuläre therapeutische Herausforderung bedarf einer breiten operativen Expertise, einer individuellen Entscheidungsfähigkeit hinsichtlich des weiteren therapeutischen Managements auch bezüglich konservativer Nachsorge und gutachterlicher Einschätzung der Verletzungsfolgen.

Abstract

Osteitis of the femoral shaft needs to be recognized and diagnosed quickly so that the treatment can be selected, because a medullary phlegmon can develop so rapidly. The last few decades have seen the emergence of the present concept of serial revision combined with the wide range of stabilising osteosynthetic implements and both local and intramedullary antiseptic and antimicrobic treatments, supported by systemic antibiotics and specific conditioning of the soft tissue; this has led to a significant reduction of the number of cases with progression to the chronic form and its dreaded secondary complications, which can even take the form of fistular carcinoma in the most severe cases. Treatment of each case is a challenge to the surgeon, demanding wide-reaching operative expertise, the ability to make and take responsibility for decisions on further treatment, including conservative aftercare, and the ability to make an expert appraisal of the likely sequelae.
