Zusammenfassung
Pseudarthrosen des Humerus treten mit einer Häufigkeit von 1–15% auf. Zahlreiche operative Stabilisierungsverfahren mit sehr unterschiedlichen Heilungsraten werden angegeben, wobei meist die Plattenosteosynthese favorisiert wird. Vom 1.1.1995–30.6.2001 wurden 47 Patienten mit Humeruspseudarthrosen (3 proximal, 36 diaphysär und 8 distal) operativ mittels Plattenosteosynthese (26 konventionelle, 21 winkelstabile) und autologer Spongiosaplastik stabilisiert. Postoperativ zeigten sich 4 Radialisparesen sowie ein Ausriss einer LCDCP aus dem Schaft, der zur Reosteosynthese zwang. Tiefe Infekte oder Probleme mit der Spongiosaentnahme traten nicht auf. 45 Patienten konnten nach durchschnittlich 29,7 Monaten nachuntersucht werden. Alle Pseudarthrosen waren knöchern konsolidiert. Die 4 Radialisparesen hatten sich alle komplett zurückgebildet. 27 von 30 Patienten waren in ihren alten Beruf zurückgekehrt. Unsere Ergebnisse zeigen, dass Pseudarthrosen des Humerus mittels Plattenosteosynthese und autologer Spongiosaplastik mit akzeptabler Komplikationsrate in den meisten Fällen sicher zur Ausheilung gebracht werden können, wobei winkelstabilen Implantaten aufgrund ihrer biomechanischen Vorteile der Vorzug gegeben werden sollte.
Abstract
Nonunion of the humerus occurs in 1–15% of cases. Numerous procedures have been proposed for stabilisation of such nonunions, with very different success. Plating is the method most commonly applied. From ¶1 January 1995 to 30 June 2001 a total of ¶47 patients with nonunions of the humerus (3 proximal, 36 diaphysial and 8 distal) were stabilised by plating (conventional plating in 26 and with an internal fixator and locking screws in 21) and autogenous cancellous bone grafting. Postoperatively 4 radial nerve palsies and 1 failure of an LCDCP requiring revision osteosynthesis were seen. We observed no deep wound infections and no donor site morbidity from bone graft harvesting. Follow-up investigations were possible in 45 patients after a mean of 29.7 months. All nonunions had healed. The 4 radial nerve palsies had completely regressed. Most (27 of 30) patients had returned to work in their previous occupations. In conclusion, we found plating and autogenous cancellous bone grafting led to bony consolidation of humeral nonunions in most cases, with an acceptable complication rate. Preference should be given to internal fixators with locked screws because of their better biomechanical characteristics.
