Periprothetische Femurfrakturen bei einliegender Hüfttotalendoprothese

Zusammenfassung

Bei zunehmender Zahl an implantierten Hüfttotalendoprothesen ist in steigendem Maß mit periprothetischen Frakturen zu rechnen. In der Literatur wird die operative Therapieform kontrovers diskutiert. Es werden sowohl die Klassifkation als auch die Genese als auch die Diagnostik der Frakturen dargelegt. Die therapeutischen Optionen in Form der konservativen Therapie und auch der Plattenosteosynthese, auch in Kombination mit einem zementierten Schaftwechsel, werden dargestellt. Des Weiteren wird auf die Möglichkeit der Implantation einer Tumorprothese eingegangen. Besonderheiten der Nachbehandlung sowie der Operationsaufklärung werden erläutert. Letztendlich wird auf die besondere Problematik der Begutachtung eingegangen.

Abstract

With the increasing frequency of hip arthroplasty we have to reckon with more periprosthetic fractures. A review of the literature reveals controversy about the operative treatment of such fractures. Their origin and classification and the appropriate diagnostic procedures are presented. The management includes conservative treatment, plate osteosynthesis – in some cases associated with a new, cemented, femoral shaft, and implantation of a tumor prosthesis. The postoperative treatment and preparation of patients for informed consent are discussed in some detail, and the particular problems that can be encountered in the preparation of expert reports are commented on.
