Klinische Erfahrungen mit der Knorpelzelltransplantation

Zusammenfassung

Die bisherigen Verfahren zur biologischen Rekonstruktion eines vollschichtigen Knorpelschadens haben ein begrenztes Indikationsspektrum. Nur durch die autologe Chondrozytentransplantation (ACT) kann hyalinartiges Knorpelgewebe zur Defektdeckung gebildet werden, auch die Behandlung größerer Knorpeldefekte (>3,5 cm²) ist möglich. Neben der exakten Beachtung des Indikationsspektrums und der korrekten operativen Durchführung ist die Qualitätsprüfung der Zellkulturen von größter Wichtigkeit. Insbesondere sollte eine quantitative Transkriptionskontrolle knorpelrelevanter Markergene (wie z. B. Kollagen Typ II) erfolgen. Die ACT wurde zwischen 1999 und 2000 bei 30 Patienten angewendet. Die Nachuntersuchungen ergaben eine deutliche Reduktion der Beschwerdesymptomatik. Eine Kontrollarthroskopie zur Beurteilung des Transplantats konnte bei 10 Patienten erfolgen.

Abstract

The indications for current therapies directed at biological reconstruction of a full-thickness cartilage defect are limited, because of minor mechanically stability. However, autologous transplantation of chondrocytes (ACT) can overcome this limitation, as by this means the defect is covered with hyaline-like cartilage tissue; it is the only therapy option for larger defects (>3.5 cm²). Careful patient selection with strict insistence on the known indications, skilful surgery, and quality tests on the chondrocytes cultured in vitro are of extreme importance for the result. Investigation of the quantitative expression of indicator genes in cultured chondrocytes (e.g. collagen type II) is a specific test that is an essential component of quality assurance. Between 1999 and 2000, ACT was performed ¶in 30 patients at the BG Trauma Center in Tübingen. Clinical follow-up has shown a reduction in such symptoms as pain and swelling. A second-look-arthroscopy for engraftment monitoring was performed in 10 patients.
