Konservative Behandlung von Unterschenkelfrakturen

Zusammenfassung

Die Unterschenkelschaftfraktur ist ein repräsentatives Beispiel für die Entwicklung der modernen Frakturbehandlung, wobei die fortschreitende Entwicklung der Osteosynthesetechnik einerseits die konservative Indikation kontinuierlich zurückgedrängt, andererseits aber auch zum Verständnis der konservativen Frakturbehandlung und der pathogenetischen Bedeutung des Weichteilschadens als Auslöser schwer wiegender Komplikationen beigetragen hat. Innerhalb des breiten Spektrums der Frakturformen und Begleitverletzungen hat das konservative Verfahren immer noch einen berechtigten Stellenwert, gerade bei den einfachen und stabilen, geschlossenen Unterschenkelfrakturen. Es erfordert jedoch das Beherrschen der konservativen Behandlungsmaßnahmen, insbesondere solide Grundkenntnisse der konventionellen Extensions- und Gipstechniken sowie auch der zunehmend favorisierten funktionellen Brace-Behandlung, um behandlungsspezifische Fehlerquellen und Komplikationen zu minimieren und letztendlich dieser Behandlungsalternative ihren Erfolg zu garantieren.

Abstract

Fracture of the tibial shaft provides a good illustration of the development of modern fracture treatment: the progressive advances in techniques of osteosynthesis have continuously narrowed the range of indications for conservative treatment on the one hand, while nonetheless also contributing to our understanding of conservative fracture treatment and of the pathogenetic importance of soft tissue damage as the source of severe complications. Within the broad spectrum of the various types of fracture and concomitant trauma there are undoubtedly still indications for conservative treatment, specifically in the case of simple, stable closed fractures of the lower leg. However, familiarity with and proficiency in the methods of conservative treatment, and in particular the conventional techniques applied in extension and plaster casting, and also the functional bracing that is increasingly favoured, so as to exclude therapy-specific sources of failure and complications and ultimately to ensure that this alternative treatment method will lead to a successful outcome.
