Zusammenfassung

Die Bedeutung einer festen Verbindung zwischen Schraube und Implantat haben schon die Pioniere der Plattenosteosynthese erkannt. Für verschiedene Körperregionen erfolgte die Konstruktion einer im Plattenloch sich selbst aufbauenden Winkelstabilität. Diese basiert auf einem Reibschluss von verschieden harten Reintitanmaterialien. Durch das Eindrängen eines Gewinde tragenden Schraubenkopfs aus einem härteren Reintitan in ein Plattenloch, welches aus einem weicheren Titan besteht, kommt es zu einer dauerhaft festen Verbindung. Diese Form der Winkelstabilität erlaubt eine multidirektionale Schraubenpositionierung. Nach dem gleichen Prinzip wurde ebenfalls eine verbesserte Verriegelung für Marknägel entwickelt. Derartige Implantate mit Winkelstabilität besitzen Vorteile gegenüber nicht winkelstabilen Implantaten. Sie ermöglichen schonende, minimalinvasive, operative Methoden und führen nach jetziger Einschätzung zu günstigeren Heilungsverläufen.

Abstract

The importance of a tight connection between screws and implant was well recognized by the pioneers of plate fixation. For several body regions, titanium plates into which screws with self-tapping screw heads can be inserted have been developed. Since for the plates softer titanium is used than the more rigid titanium used for the screws, an implant with angular stability is created and the screws can be inserted in different angles. The same principle has been applied to the development of an improved locking system for intamedullary nails. Biomechanical research has demonstrated the advantages of locked-plate systems, over plates without angular stability. They can be inserted by a minimally invasive surgical approach, and fracture healing is improved.
