Zusammenfassung

Frakturen im Bereich des oberen Sprunggelenks zählen mit einer Inzidenz von etwa 1‰ in der europäischen Bevölkerung zu den häufigsten Verletzungen, wobei sich 40% dieser Verletzungen beim Sport im Sinn eines indirekten Traumas durch Stolpern oder Umknicken ereignen. Die heute gebräuchlichen Einteilungen der Sprunggelenkfrakturen gründen auf die Arbeiten von Ashurst und Bromer 1922, Lauge-Hansen 1948 und Danis 1949 sowie Weber 1966. Die Standardtherapieformen haben sich in den letzten ¶40 Jahren nur wenig verändert. Während stabile, nichtdislozierte Frakturen konservativ behandelt werden können, sollten bei ¶allen dislozierten Frakturen eine möglichst anatomische Rekonstruktion und stabile interne Osteosynthese der Fibula, der Syndesmose, des Innenknöchels und des hinteren Volkmann-Dreiecks erfolgen. Nur so können die Gefahr einer Dystrophie minimiert und Folgeeingriffe bis hin zur Versteifungsoperation vermieden werden.

Abstract

Fractures of the ankle joint are among the most frequent injuries sustained in the European region, with an incidence of 1‰. In 40% of cases the ankle joint is injured during sport by indirect rotational, translational and axial forces. The classification system now most frequently used is based on those published by Ashurst and Bronner in 1922, Lauge-Hansen in 1948, Danis in 1949 and Weber in 1966. The treatment has not changed substantially in the last 40 years. Restoration of the normal anatomy of the joint is the basic goal. While stable fractures that are not displaced can be treated by conservative methods, open anatomical reduction with stable internal fixation of the fibula, syndesmosis, medial malleolus and distal posterior tibial margin is mandatory in unstable displaced fractures. This treatment is the only way of avoiding dystrophy and subsequent operations, including arthrodesis of the ankle joint.
