Titan-Fixateur-interne-Systeme mit multidirektionaler Winkelstabilität im Unterschenkel- und Fußbereich

Zusammenfassung

Von September 1997–Oktober 2001 wurden bei insgesamt 130 Patienten 136 Operationen mit einem Titanfixateur interne (TiFix®) am Unterschenkel durchgeführt. Bei 88 Patienten erfolgte die Stabilisierung von 94 Frakturen, wobei in 65 Fällen ein Umstieg vom Fixateur externe stattfand. 29 Frakturen wurden primär versorgt. Des Weiteren wurden 37 Patienten mit Pseudarthrose sowie ¶5 Patienten mit Fehlstellungen korrigiert. Zum Nachuntersuchungszeitpunkt sind 102 Behandlungen abgeschlossen. Ein Implantatversagen fand sich bislang nicht. Eine vollständige Durchbauung konnte in allen Fällen erreicht werden, davon einmal durch Umstieg auf einen Fixateur externe. In 108 Fällen erfolgte die Implantation minimalinvasiv, in 67 Fällen wurde zusätzlich eine autologe Spongiosaplastik mit Einlage einer Refobacin-Pallacos-Kette durchgeführt. Postoperativ fanden sich 5 Infekte nach Pseudarthrosenrevision bei 4-maligem Keimnachweis zum Zeitpunkt des Verfahrenswechsels. Von September 1998–Dezember 1999 wurden 22 Kalkaneusfrakturen mit einem TiFix® operativ stabilisiert. Der Tubergelenkwinkel konnte durchschnittlich um 16° verbessert werden. Ein sekundärer Stellungsverlust des zentralen Fragments oder ein tiefer Infekt fanden sich in keinem Fall. Weitere 7 Arthrodesen des unteren Sprunggelenks und 6 des Talonavikulargelenks heilten aus.

Abstract

Between September 1997 and October 2001, a total of 136 lower leg operations using a titanium plate system allowing multidirectional angular stability (TiFix) were performed on 130 patients. In all, 94 fractures were stabilized in 88 patients; in 65 cases these had initially been treated by external fixation before the TiFix system was substituted. Only in 29 fractures was the TiFix system used as the primary treatment. In addition, 37 patients with pseudarthrosis and ¶5 patients with misalignment underwent these operations as revision surgery. In 102 cases the treatment was concluded at the time of the follow-up examination. No hardware failures have so far been observed. Bone healing was complete in all cases, but in 1 case it was necessary to change to an external fixator. Minimally invasive osteosynthesis was performed in 108 cases, and in 67 cases autologous cancellous bone was introduced at the fracture site and local antibiotics were administered (Refobacin-Palacos chain). In 4 of the 5 cases of pseudarthrosis positive bacterial evidence was found when the procedure was changed, and infection of the bone and soft tissue developed. The implants were promptly removed, and spontaneous healing of the soft tissue followed. From September 1998 through December 1999, 22 calcaneus fractures were stabilized with the TiFix angularly stable plate system. The tuber angle was improved by an average of 16°. Secondary dislocation of the central fragment did not occur in any of these cases. TiFix plate systems were also used in 7 cases of arthrodesis of the subtalar joint and 6 of arthrodesis of the talonavicular joint, leading to complete bone healing; no postoperative complications were observed in these cases.
