Titan-Fixateur-interne-Systeme im Bereich der oberen Extremitäten

Zusammenfassung

Von Oktober 1998–Dezember 2000 wurden 11 Humeruskopffrakturen, 5 Humerusschaftfrakturen, 3 Unterarmfrakturen, 16 Humerusschaftpseudarthrosen und 21 Unterarmpseudarthrosen mit einem winkelstabilen Titan-Fixateur-interne-System (TiFix®) operativ versorgt. Für die proximalen Humerusfrakturen wurde ein TiFix® entwickelt, am Humerusschaft kamen der TiFix® für den Unterschenkel und eine Spezialanfertigung zum Einsatz. Für den Unterarm wurden ein TiFix® mit der Dimensionierung einer schmalen LCDCP sowie Implantate für den proximalen und distalen Radius entwickelt. Bei den Humeruskopffrakturen kam es zu einem tiefen Infekt, der nach Materialentfernung abheilte, und zu 2 Plattenausrissen aus dem Knochen. Bei den Humerusschaftpseudarthrosen zeigten sich 2 passagere Radialisparesen. Eine Knochenheilungsstörung trat einmal bei einer proximalen Ulnapseudarthrose auf.

Abstract

From October 1998 to December 2000, ¶11 fractures of the proximal humerus, 5 fractures of the diaphysial humerus, 3 fractures of the forearm, 16 pseudarthroses of the diaphysial humerus and 21 pseudarthroses of the forearm were stabilized with titanium internal fixator systems (TiFix® ). A new system was developed for use in the proximal part of the humerus; in the diaphysial part of the humerus we used the TiFix® designed for the tibia, and in 1 case a custom-made TiFix® system. For the forearm we developed a TiFix® with the same dimensions as a small LCDCP, and two TiFix® systems were designed for the proximal and distal radius. Implantation of the system for treatment of the fractures of the proximal humerus were followed bydeep wound infection in 1 case, which abated without further complications after removal of the implant, and loosening of the TiFix® system from the bone in 2. In 2 cases of pseudarthrosis of the diaphysial humerus we saw temporary paresis of the radial nerve. We found failure of bony consolidation in 1 case of pseudarthrosis of the proximal ulna.
