Posttraumatisches Karpaltunnelsyndrom

Zusammenfassung

Das Karpaltunnelsyndrom stellt die häufigste periphere Kompressionneuropathie dar. Durch die anatomische Lage des N. medianus im osteofibrösen Karpalkanal führen Verletzungen der knöchernen und Bandstrukturen im Bereich des distalen Unterarms sowie des Carpus direkt oder durch Ausbildung von Hämatomen und Infektionen bzw. sekundärer Vernarbungen zu einer Druckeinwirkung auf diesen Nerv. Hieraus resultieren die kurzfristig nach dem Unfallereignis auftretenden bzw. chronischen Beschwerden des Karpaltunnelsyndroms. Nach eingehender klinischer Untersuchung und der elektrophysiologischen Befundabklärung sollte schnellstmöglich die offene Spaltung des Retinaculum flexorum erfolgen.

Abstract

Carpal tunnel syndrome is the most common peripheral compression neuropathy. Owing to the anatomical position of the median nerve in the osteofibrous carpal tunnel, injuries to the bony and ligamentous structures in the region of the distal part of the lower arm and to the wrist mean that pressure is exerted on this nerve, either directly or as the result of haematomas and infections or secondary scarring. This in turn gives rise to the symptoms of carpal tunnel syndrome, which can either be short-lived for a short period starting shortly after the causal event or become chronic. A thorough clinical examination and interpretation of the results of electrophysiological testing should be followed as quickly as possible by open surgery to divide the flexor retinaculum.
