Zusammenfassung

Die angemessene Behandlung der Radiusköpfchenfrakturen erfolgt nach korrekter Klassifikation unter Berücksichtigung von Begleitverletzungen und dem Anspruch des Patienten. Die konservativ-frühfunktionelle Behandlung erzielt bei nicht dislozierten Mason-I-Frakturen überwiegend gute bis sehr gute Ergebnisse. Mason-II-Frakturen sind in der Regel eine Domäne der operativen Therapie mit Rekonstruktion der Gelenkfläche und postoperativ funktioneller Behandlung mit guten Ergebnissen. Bei Mason-III-Frakturen muss die Indikation bezüglich Resektion oder Ersatz in Abhängigkeit von Begleitverletzungen und Gelenkstabilität gestellt werden. Nach frühzeitiger Resektion bei Gelenkstabilität bzw. nach frühzeitigem Ersatz bei Begleitverletzungen mit Instabilität werden gute und befriedigende Ergebnisse erzielt. Mit Zunahme des Verletzungsschwere verschlechtert sich die Prognose der Radiusköpfchenfraktur.

Abstract

Correct classification of any radial head fracture is required before it is possible to decide what treatment will be best. However, additional injuries and the personal wishes of the patient should also be taken into account. With early functional treatment good and excellent results are achieved in most nondislocated Mason type-I fractures. In the case of Mason type-II fractures reconstructive surgery is generally performed. If reconstruction of the joint surface is possible good results are reported. Mason type-III fractures demand full consideration of all the treatment options, including resection and replacement of the radial head. Depending on the additional injuries present, early resection or, in the case of instability, prosthetic replacement of the radial head yields good or moderate results. The prognosis of radial head fractures depends primarily on the grade of fracture and the gravity of the accompanying injuries.
