Zusammenfassung
Die neueren klinisch einsetzbaren Knochenersatzmaterialien sowohl bovinen als auch synthetischen Ursprungs, Letztere insbesondere als Kalziumphosphatzemente, lassen ein erweitertes Reaktionsspektrum mit höherer biologischer Integration, v. a. Degradation, und strukturbedingt definiertere Modifikationen der frakturbedingten Belastungsmöglichkeiten im Vergleich zu früheren Ersatzmaterialgenerationen erkennen. Die applikations- und belastungsspezifischen Möglichkeiten eröffnen einerseits ganz neue, insbesondere minimalinvasive Indikationen bei den in situ aushärtenden Kalziumphosphatzementen, lassen andererseits aber auch bei den Kollagen erhaltenden bovinen Materialien die wesentliche Anforderung der Osteointegration und der Remodellierung erkennbar auf dem Vormarsch erscheinen. Die vorliegende Arbeit vergleicht das Einsatz-, Belastungs- und biologische Degradationsprofil verschiedener, klinisch vor Ort eingesetzter struktureller Knochenersatzmaterialien und zeigt den künftigen spezifischen Entwicklungshorizont auf.
Abstract
The new bone substitutes now available for clinical use, both synthetic and of bovine origin, especially calcium-phosphate cements in the former case, allow realization of an extended spectrum of reaction with higher biological integration, in particular degradation, and structurally defined modifications of the loading capacities determined by the fracture. The options specific to the different applications and the mechanical loading allow the use of completely new indications, in particular for minimally invasive techniques with the calcium phosphate cements hardening in situ, while on the other hand the collagen-preserving bovine materials are much improving the degree to which the main demand for osteointegration and remodeling is met. This article compares the profile of indications, loading conditions and biological degradation of different structural bone substitutes used in this clinic and reveals the improvements that can be expected in the future.
