Zusammenfassung
Zielsetzung
Die chirurgisch forcierte Gaumennahterweiterung kann sowohl mit zahngetragenen Plattenapparaturen als auch mit knochengetragenen Distraktoren durchgeführt werden. Bei der klinischen Anwendung beider Verfahren haben wir über die Jahre systemabhängige Mängel festgestellt, sodass wir in Zusammenarbeit mit der Firma Normed, Tuttlingen, einen neuen, universell einsetzbaren knochengetragenen Distraktor (MWDTM, Maxillary Widening Device) entwickelt haben. Die vorliegende Arbeit beschreibt unsere ersten Erfahrungen im klinischen Einsatz dieses Gerätes.
Probanden und Methoden
Der Einsatz des MWDTM erfolgte bei insgesamt 20 dysgnathen Patienten mit transversaler Oberkieferkompression in Verbindung mit einer nach Glassmann modifizierten Le-Fort-I-Osteotomie über einen Zeitraum von zwölf Monaten. Außerdem wurde das MWDTM bei zwei Spaltpatienten mit uni- bzw. bilateraler Lippen-Kiefer-Gaumenspalte zur Korrektur der disloziert stehenden Oberkiefersegmente sowie bei einem Kind mit bilateraler Mikrosomie eingesetzt.
Ergebnisse
Das neu entwickelte MWDTM stellte sich in der klinischen Anwendung als kompakter, sicherer und handlicher knochengetragener Distraktor heraus. Er ist sowohl bei frühadulten als auch bei Syndrompatienten einsetzbar. In der frühadulten Patientengruppe betrug das Durchschnittsalter 19,5 Jahre und die durchschnittliche Weitung der Maxilla 7,6 mm. Die Komplikationsrate lag bei 25%. Rezidive wurden nicht beobachtet.
Schlussfolgerung
Das MWDTM erweist sich als eine benutzerfreundliche, stabile und zuverlässige Dehnapparatur, mit der sich gute Behandlungsergebnisse erzielen lassen.
Abstract
Aim
Conventional devices for surgical assisted rapid palatinal expansion are either tooth or bone borne. During clinical application over years, system dependant deficiencies, using either device became apparent. So together with Normed, Tuttlingen, we developed a new bone borne distractor (MWDTM, Maxillary Widening Device). In this article we introduce this distractor and present our first experiences in using this device.
Patients and Methods
In this study, 20 patients suffering from maxillary arch compression with anterior crowding were treated using MWDTM in combination with Glassman's modified Le-Fort-I-osteotomy. Additionally the MWDTM was inserted in two cleft lip and palate patients, as well as one patient with bilateral microsomy.
Results
The MWDTM turned out to be a safe, easy to handle and reliable bone borne distractor leading to excellent results. The MWDTM can be used in early adult as well as in syndromal patients. In early adult patients the complication rate averaged 25%, widening averaged 7,6 mm. Relaps was not discovered.
Conclusion
The MWDTM turned out to be a reliable, stable and user-friendly device leading to excellent results.
