Zusammenfassung
Die diabetische periphere Polyneuropathie ist eine Folgekomplikation des Diabetes mellitus und hauptverantwortlich für die Entwicklung eines diabetischen Fußsyndroms. Hauptursache ist die schmerzlose Druckläsion. Grundvoraussetzung zur Wundheilung ist die vollständige und dauerhafte Druckentlastung. Die besten Ergebnisse werden mit am Fuß fixierten und nicht abnehmbaren Druckentlastungssystemen erzielt. Der Total Contact Cast (TCC) gilt weiterhin als Goldstandard. Bei praktisch einfacher Umsetzung werden auch sehr gute Ergebnisse mit der lokalen Filzpelottierung erreicht – entscheidend ist die dauerhafte Fixierung am betroffenen Fuß.
Abstract
Diabetic peripheral polyneuropathy is a secondary complication of diabetes mellitus and is mainly responsible for the development of a diabetic foot syndrome. The main cause is a painless pressure lesion. The basic prerequisite for wound healing is the complete and permanent offloading. The best results are achieved with pressure relief systems with nonremovable fixation on the foot. The total contact cast (TCC) is still considered the gold standard. With a simple and practical implementation, very good results are also achieved with local felted foam indentation and the decisive factor is the permanent fixation to the affected foot.
