Zusammenfassung
Die Initiative „Gemeinsam Klug Entscheiden“ hinterfragt übliche Versorgungspraktiken in Deutschland und erstellt unabhängig von Leitlinien fachspezifische Empfehlungen mit dem Ziel, Über- und Unterversorgung in der Medizin zu vermeiden. Für die Gefäßchirurgie wurden im Teilgebiet der peripheren arteriellen Verschlusskrankheit bisher keine Empfehlungen formuliert. Dennoch besteht auch bei der Diagnostik und Therapie von PAVK-Patienten in Deutschland hinsichtlich Über- und Unterversorgung ein Diskussionsbedarf. Insbesondere konservative Therapiemaßnahmen wie das Gehstreckentraining, die medikamentöse Sekundärprophylaxe, eine stadiengerechte Behandlung sowie die Verwendung von Klassifikationssystemen zur Einschätzung der Krankheitsschwere und der Verschlussanatomie finden in den Augen der Autoren im „gefäßchirurgischen“ Alltag oftmals nicht regelhaft Beachtung. Im vorliegenden Artikel werden diese in Form von Positiv- und Negativempfehlungen zusammenfassend dargestellt und diskutiert.
Abstract
Choosing wisely is an international campaign that questions routine treatment practices in Germany and formulates discipline-specific recommendations independently from the guidelines, with the aim of avoiding overtreatment and undertreatment. For the field of vascular surgery no recommendations have so far been formulated for the branch of peripheral arterial disease. Nevertheless, there is a need for discussion also in the diagnostics and treatment of patients with peripheral arterial disease in Germany with respect to undertreatment and overtreatment. In the opinion of the authors, particularly conservative treatment modalities, such as exercise training, secondary medication prophylaxis, stage-related treatment, the use of classification systems for estimation of disease severity and the anatomy of the occlusion are often regularly not given adequate consideration in routine vascular surgery. In this article these aspects are presented, discussed and summarized in the form of positive and negative recommendations.
