Zusammenfassung
Hintergrund
Bei Patienten mit peripherer arterieller Verschlusskrankheit (PAVK) kommt es zu einer ischämiebedingten Myopathie mit Veränderung der Mitochondrienfunktion. Mit hochauflösender Respirometrie (HRR) kann die Mitochondrienfunktion untersucht werden.
Fragestellung
Überblick über In-vivo- und In-vitro-Untersuchungsmethoden der Mitochondrienfunktion. Ausführliche Beschreibung der Methodik der HRR. Darstellung der mitochondrialen Veränderungen bei PAVK.
Material und Methode
Für die HRR wird eine Muskelbiopsie von PAVK-Patienten mit Indikation zur invasiven Behandlung oder von gesunden Probanden entnommen. Der Mitochondriengehalt im M. gastrocnemius und im M. vastus lateralis wird durch die Citrat-Synthase-Aktivität bestimmt. Die HRR zeigt die Aktivität der mitochondrialen Atmungskette durch Messung der Abnahme der Sauerstoffkonzentration in der Reaktionskammer. Durch Zugabe von Inhibitoren oder Substraten können die einzelnen Komplexe der Atmungskette untersucht werden.
Ergebnisse
Bei Patienten mit PAVK im Stadium IIb nach Fontaine kommt es im Vergleich zur gesunden Kontrollgruppe zu einem verminderten Mitochondriengehalt im Versorgungsgebiet der kompromittierten Arterie bei gleichzeitig erhöhter Aktivität. Diese Veränderungen sind durch Revaskularisation reversibel.
Schlussfolgerungen
HRR erlaubt die differenzierte, quantitative Untersuchung der mitochondrialen Atmungskette. Bei PAVK kommt es zu pathologischen Veränderungen der Mitochondrienfunktion, die in vitro trotz Überangebot von Sauerstoff und Substraten bestehen und die durch eine erfolgreiche Revaskularisation reversibel sind. Die HRR kann zur Aufklärung der Pathophysiologie der PAVK beitragen.
Abstract
Background
Patients with peripheral arterial disease (PAD) develop ischemia-induced myopathy with alteration of mitochondrial function. High-resolution respirometry (HRR) can be used to study mitochondrial function.
Objective
Overview of in vivo and in vitro investigation methods to study mitochondrial function. Detailed description of the HRR procedure. Portrayal of mitochondrial alterations in PAD.
Material and methods
For HRR a muscle biopsy is taken from patients with PAD with indications for invasive treatment or from healthy volunteers. The mitochondrial content in the gastrocnemius and vastus lateralis muscles is determined by citrate synthase activity. The HRR detects the activity of the mitochondrial respiratory chain by measuring oxygen depletion in the reaction chamber. Individual components of the respiratory chain can be examined by adding inhibitors or specific substrates,
Results
In comparison to healthy volunteers, patients with PAD Fontaine stage 2b show a reduced amount of mitochondria with concurrently increased mitochondrial activity in the area supplied by the compromised artery. These changes are reversible by revascularization.
Conclusion
The HRR enables a differentiated quantitative investigation of the mitochondrial respiratory chain. A PAD leads to pathological changes in mitochondrial function, which persist in vitro despite excess oxygen and substrates and which are reversible with successful revascularization. The HRR can contribute to clarification of the pathophysiology of PAD.
