Zusammenfassung
Ziel
In der vorliegenden Übersicht soll zur evidenzbasierten Versorgung der spontanen Carotis- und Halsarteriendissektion anhand der aktuellen Leitlinien und einer Literaturrecherche Stellung genommen werden.
Methode
Literaturrecherche in PubMed für die Jahre 2010 bis 2019 unter den Schlüsselwörtern „carotid artery dissection [Title]“. Die A. vertebralis wurde miteinbezogen, da in den meisten Artikeln beide Gefäße betrachtet werden. Es wurden nur isolierte Halsarteriendissektionen berücksichtigt und Mitbeteiligung im Rahmen von Aortendissektionen ausgeschlossen.
Ergebnisse
Spontane Dissektionen der Carotiden und Vertebralarterien werden zwar nur für 2 % aller ischämischen Schlaganfälle verantwortlich gemacht, aber sie haben dennoch klinische Bedeutung, da sie 10–25 % aller ischämischen Schlaganfälle bei jüngeren Patienten und solchen mittleren Alters verantworten. Die durchschnittliche Inzidenzrate wird für die Dissektion der Carotis interna (ICA) mit 1,72 auf 100.000 Einwohner, für die Dissektion der Vertebralarterien mit 0,97 auf 100.000 Einwohner geschätzt. Ob bei Patienten mit ICA-Dissektion und Schlaganfall eine endovaskuläre Therapie der intravenösen Thrombolyse überlegen ist, muss offenbleiben. In einer vergleichenden Untersuchung von 668 spontanen Dissektionen der ICA und 302 Dissektionen der Vertebralarterien wiesen Patienten mit ICA-Dissektion zwar seltener als solche mit Dissektion der Vertebralarterien einen ischämischen Schlaganfall auf (52,2 % vs. 69,5 %), aber die Schlaganfälle zeigten bei ICA-Dissektion einen signifikant höheren Schweregrad. Entsprechend war die Prognose der Schlaganfallpatienten mit Vertebralarteriendissektion signifikant besser als die von Patienten mit ICA-Dissektion.
Fazit
Eine antithrombotische Therapie zur Schlaganfallprophylaxe mit Thrombozytenfunktionshemmern oder Antikoagulation mit Heparin/Warfarin wird bei Personen mit der Diagnose einer extrakranialen Carotis- oder Vertebralarteriendissektion empfohlen. Hinsichtlich der optimalen Dauer einer antithrombotischen Therapie fehlt Evidenz. Ebenfalls gibt es zurzeit nicht genügend Evidenz, um eine Empfehlung hinsichtlich der Anwendung von direkten oralen Antikoagulantien bei Patienten mit arteriellen Dissektionen treffen zu können.
Abstract
Objective
In the present review, the current guidelines and a literature review should be used to provide evidence-based care for carotid and vertebral artery dissection.
Method
The literature was searched in PubMed for the years 2010 to 2019 under the keywords “carotid artery dissection [Title]”. The vertebral artery was included because most publications consider both vessels. Only isolated cervical artery dissections were considered and participation in aortic dissections was excluded.
Results
Although spontaneous dissections of the carotid and vertebral arteries are thought to account for only 2% of all ischemic strokes, they are still of clinical significance, as they account for 10–25% of all ischemic strokes in younger and middle-aged patients. The average incidence for dissection of the internal carotid artery (ICA) is estimated to be 1.72 per 100,000 inhabitants and that for dissection of the vertebral arteries 0.97 per 100,000 inhabitants. Whether endovascular therapy is superior to intravenous thrombolysis in patients with ICA dissection and stroke is uncertain. In a comparative study of 668 spontaneous dissections of the ICA and 302 dissections of the vertebral arteries, patients with ICA dissection showed ischemic stroke less frequently than those with dissection of the vertebral arteries (52.2% vs 69.5%), but the strokes showed significantly greater severity in ICA dissection. Accordingly, the prognosis of patients with a stroke and vertebral artery dissection was significantly better than that of patients with ICA dissection.
Conclusion
Antithrombotic therapy with antiplatelet therapy or anticoagulation with heparin/warfarin for stroke prevention is recommended in patients with a diagnosis of extracranial carotid or vertebral artery dissection. There is a lack of evidence regarding the optimal duration of antithrombotic therapy. Also, there is currently insufficient evidence to make a recommendation regarding the use of direct oral anticoagulants in patients with arterial dissections.
