Zusammenfassung
Kompetenz bezieht sich nicht nur auf Wissen und Fertigkeiten einer Person, sondern auch auf deren Handlungsfähigkeit im klinischen Alltag, insbesondere unter Stress. Deshalb sollten wir die Kompetenzprofile der Mitarbeiter kennen. Die shuntchirurgischen Behandlungskonzepte beruhen heute mehr auf Eminenz als auf Evidenz, weil die Zahl randomisierter Studien beschränkt ist. Es können etablierte Simulationskurse belegt werden, um Fertigkeiten in der Shuntchirurgie zu trainieren. In diesem Artikel werden am Beispiel der Shuntchirurgie „entrustable professional activities“ (EPAs) formuliert. Mit diesen „anvertraubaren professionellen Tätigkeiten“ können Kernkompetenzen im klinischen Alltag abgebildet werden. Diese EPAs erlauben es dem Vorgesetzten einzuschätzen, wie eigenverantwortlich eine Person arbeiten kann. Wiederholt man diese Beurteilung über die Zeit, können Veränderungen des Kompetenzprofils eines Kandidaten über eine längere Periode beobachtet werden.
Abstract
Competence refers not only to the knowledge and skills of a person but also to the capacity to perform properly in the daily clinical routine, particularly in critical and stressful situations. Therefore, it is important to know the competency profile of a person. In vascular access surgery the evidence level is low and eminence often prevails because the number of randomized studies is limited. High fidelity simulation courses enable adequate skills training in the art of arteriovenous fistula and graft creation. In this article, the concept of “entrustable professional activities” (EPAs) is explored to show how this tool monitors progress in clinical competences of vascular access surgeons in training. These EPAs can be used to reflect the core competences in the clinical routine. They enable the superiors to estimate how independently a person works. If this assessment is repeated over time, changes in the competence profile of a candidate can be observed over a longer time period.
