Zusammenfassung
Das Behçet-Syndrom (BS) (auch Admantiades-Behçet-Syndrom oder Morbus Behçet) ist eine seltene, systemische Vaskulitis, die durch orale Aphthen, Genitalgeschwüre, entzündliche Augenveränderungen und andere systemische Manifestationen gekennzeichnet ist. Es kommt am häufigsten in der eurasischen Bevölkerung entlang der „Seidenstraße“ vor, die sich von Ostasien bis zum Mittelmeerraum erstreckt. Die Ursachen für das BS sind unbekannt: Es werden sowohl autoinflammatorische als auch autoimmunologische Prozesse angenommen, die durch einen infektiösen oder umweltbedingten Auslöser bei genetisch veranlagten Personen zum entzündlichen Krankheitsbild führen.
Entsprechend der „Chapel-Hill-Nomenklatur“ für Vaskulitiden wird das BS als Vaskulitis variabler Gefäßkaliber klassifiziert. Es sind sowohl Arterien als auch Venen betroffen. Dies führt u. a. zu einem sehr variablen Krankheitsgeschehen, das durch einen schubartigen Verlauf mit sehr unterschiedlicher Symptomatik und variabler Organmanifestationen gekennzeichnet ist. Ein lebensbedrohlicher Verlauf kann sich bei Beteiligung des Magen-Darm-Trakts, des Nervensystems und der großen Gefäße entwickeln. Entsprechend sind eine frühe Diagnose und adäquate Behandlung zu fordern. Die Therapie des BS umfasst heute ein breites Spektrum antiinflammatorischer und immunmodulatorischer Wirkstoffe mit zunehmender Bedeutung von Biologika und „small molecules“.
Abstract
Behçet’s syndrome (BS, also known as Adamantiades-Behçet syndrome or Behçet’s disease) is a rare systemic vasculitis characterized by oral aphthae, genital ulcers, inflammatory ocular changes and other systemic manifestations. It is most prevalent in the Eurasian population along the Silk Road, which stretches from East Asia to the Mediterranean area. The causes of BS are unknown: both autoinflammatory and autoimmunological processes are assumed, which lead to an inflammatory clinical picture due to an infectious or environmental trigger in genetically predisposed persons. According to the Chapel Hill nomenclature for vasculitides BS is classified as vasculitis of variable vascular caliber. Both arteries and veins are affected. This leads to a very variable disease event that is characterized by a relapsing course with very different symptoms and variable organ manifestations. A life-threatening course can develop with involvement of the gastrointestinal tract, the nervous system and large vessels. Accordingly, early diagnosis and adequate treatment are required. The therapy of BS currently includes a broad spectrum of anti-inflammatory and immunomodulatory agents with an increasing importance of biologics and small molecules.
