Zusammenfassung

Die endovaskuläre Aneurysmaausschaltung („endovascular aortic aneurysm repair“, EVAR) wird in Deutschland nahezu flächendeckend aufgrund ihrer reduzierten perioperativen Komplikations- und Mortalitätsrate als Therapie der ersten Wahl angeboten. Die offene Chirurgie wird deshalb zunehmend seltener durchgeführt und bleibt seltenen und dann oft komplexen Indikationen vorbehalten. Die Ausbildung in der offenen abdominellen Aortenaneurysma(AAA)-Chirurgie stellt eine sehr große Herausforderung in der Weiterbildung des gefäßchirurgischen Nachwuchses dar, zumal ein Zusammenhang zwischen Überleben und Operationsvolumen von Krankenhäusern und -Frequenz von Operateuren besteht. Eine Anpassung der Weiterbildungsordnung, ein Training an Simulatoren und eine Zentralisierung zumindest der offenen AAA-Chirurgie ist sinnvoll und zukünftig potenziell notwendig, um selektionierten Patienten diese Therapiealternative weiterhin mit langfristig haltbaren und guten Ergebnissen anbieten zu können.

Abstract

Endovascular aortic aneurysm repair (EVAR) is available practically nationwide in Germany as the first choice treatment for patients with abdominal aortic aneurysms (AAA) due to the reduced perioperative complication and mortality rates. Open surgery is therefore carried out less frequently and is reserved for rare and often complex indications. Nowadays, the education and training of trainees in vascular surgery in open AAA repair is challenging. There is published evidence for a strong correlation between survival and operation volume of hospitals and operation frequency of surgeons. Regulations on further education need to be adapted. Simulator training and centralization for AAA treatment, at least for open AAA repair make sense and are mandatory in the future in order to provide open surgery to selected patients with AAA as a valuable treatment alternative with durable long-term results.
