Zusammenfassung
Hintergrund
Für die erfolgreiche Implementierung eines Screeningprogramms müssen international konsentierte Kriterien erfüllt werden. Der Nutzen muss größer sein als der Schaden. Die Multimorbidität der Zielgruppe der älteren Menschen erfordert eine besondere Berücksichtigung der Lebensqualität. Weitere wichtige Qualitätskriterien bei der ethischen Nutzenbewertung eines Screeningprogramms sind die Ressourcenallokation, Qualitätssicherung und die informierte Patientenentscheidung. Grundlage einer informierten Entscheidung ist eine evidenzbasierte Gesundheitsinformation.
Material und Methode
Es erfolgte eine Bestandsaufnahme von Gesundheitsinformationen und Entscheidungshilfen (EH) über eine systematische Datenbankrecherche (PubMed, PsycINFO, PSYNDEX) und eine ergänzende Suche über Google. Identifizierte Informationen wurden hinsichtlich Zielsetzung, Nutzen-Schaden-Darstellung und Evaluation bewertet.
Ergebnisse
Es wurden 1639 Publikationen identifiziert, 8 Volltexte beurteilt. Keine EH zum AAA wurde in einer randomisiert-kontrollierten Studie überprüft. Es konnte keine relevante deutschsprachige Information oder EH identifiziert werden. Das UK National Screening Committee bietet umfangreiche Informationen an, jedoch ohne einheitliche Darstellung aller relevanten Aspekte. Beispielhaft wird gezeigt, wie eine deutschsprachige EH zum AAA aussehen könnte.
Schlussfolgerung
Sollte die Einführung eines Screeningprogramms auf AAA in Deutschland beschlossen werden, besteht Entwicklungsbedarf für eine evidenzbasierte Gesundheitsinformation oder EH, die eine informierte Patientenentscheidung unterstützt. Die Dokumentation der informierten Entscheidung sollte ein wichtiges Qualitätskriterium sein.
Abstract
Background
For implementation of a screening program specific quality criteria have to be taken into account. The benefits have to outweigh the harm. Multimorbidity of the target group of elderly people requires particular consideration of the quality of life. Further important indicators of quality for the ethical appraisal of a screening program are the allocation of resources, quality assurance and informed decision making. The core element of informed decision making is evidence-based health information.
Methods
A search was carried out for health information and decision aids (DAs) for abdominal aortic aneurysms (AAA) based on a systematic literature search in PubMed, PsycINFO, PSYNDEX and for German language material in Google. Identified information was rated with respect to the objectives, presentation of benefits and harm and evaluation of the results.
Results
A total of 1639 titles and 8 full text articles were reviewed. No AAA-related DAs were evaluated in a randomized controlled trial. The search revealed no relevant health information or DAs in the German language. The UK National Screening Committee provides extensive information material but without a standard presentation of the relevant contents. An example of an AAA-related DA for potential use in Germany is presented.
Conclusion
Implementation of an AAA screening program in Germany would require the development of evidence-based health information or DAs to support informed decision making. The documentation of the informed decision making should be a relevant parameter in quality assurance.
