Zusammenfassung
Eine 16-jährige Patientin stellte sich notfallmäßig mit einer beidseitigen Becken-Bein-Venenthrombose vor. Ursächlich wurde eine segmentale, kongenitale Hypoplasie der V. cava inferior diagnostiziert. Es wurde simultan eine venöse transfemorale Thrombektomie in Kombination mit einer ballonunterstützten Stentimplantation im Sinne einer Hybridprozedur erfolgreich durchgeführt. Im Follow-up ist die Patientin bei offenem Stent und unbeinträchtigtem venösem Abstrom nach 58 Monaten beschwerdefrei.
Bei Patienten mit Beckenvenenthrombosen sollte eine CT- oder MR-Angiographie mit genauer Darstellung des proximalen Thrombusendes durchgeführt werden. Hierbei sollte eine Hypoplasie der V. cava trotz ihrer geringen Inzidenz in Betracht gezogen werden. Wird bei selektionierten Patienten eine venöse Thrombektomie durchgeführt, ist die intraoperative Phlebographie obligat. Wird ein Strombahnhindernis diagnostiziert, ist die simultane stentgestützte Angioplastie eine Therapieoption mit guten Langzeitergebnissen.
Abstract
A 16-year-old female presented in an emergency situation with a bilateral iliofemoral vein thrombosis caused by segmental congenital hypoplasia of the inferior caval vein. This pathology was treated by transfemoral venous thrombectomy and endovascular stenting in a one-stage procedure. After a follow-up of 58 months the young patient is free of symptoms and the stent is still patent.
In patients with iliofemoral vein thromboses CT or MRI has to be performed preoperatively to localize the proximal end of the thrombus. Hypoplasia of the caval vein is a rare differential diagnosis but should always be considered. If a venous thrombectomy is performed in selected patients, an intraoperative phlebography is mandatory. An obstacle in the venous system should be treated simultaneously by stent-assisted angioplasty as it offers a therapeutic option with good long-term patency rates.
