Zusammenfassung
Die Beteiligung von Gefäßen bei nichtvaskulären Tumoren ist immer ein Hinweis auf ein fortgeschrittenes lokales Tumorwachstum. Bei gleichzeitigem Nachweis von Fernmetastasen liegt eine inkurable Situation vor. Können Fernmetastasen differenzialdiagnostisch ausgeschlossen werden, sollte eine radikale Resektion ohne Rücksicht auf anatomische Grenzen mit dem Ziel einer R0–Situation angestrebt werden. Infiltriert das Tumorgewebe Arterien- und/oder Venenabschnitte, ist eine radikale En-bloc-Resektion mit nachfolgender Rekonstruktion der Gefäße von wesentlicher Bedeutung für den therapeutischen Erfolg. In bestimmten Fällen, z. B. bei einem extravasalen Primärtumor, ist zur Therapieplanung eine interdisziplinäre Zusammenarbeit unabdingbar.
Abstract
The involvement of vessels in cases of non-vascular tumors is always an indication of advanced local tumor growth. Simultaneous evidence of distant metastases represents an incurable situation. If distant metastases can be excluded by differential diagnosis, a radical resection should be performed without any consideration of anatomical limitations to achieve a R0 situation. If the tumor infiltrates arteries or veins, a radical en bloc resection with subsequent reconstruction of the vessels is of substantial importance for therapeutic success. In certain cases, e.g. an extravascular primary tumor, interdisciplinary cooperation is absolutely necessary for therapy planning.
