Zusammenfassung
Einleitung
Der chirurgische Zugangsweg zur A. fibularis ist technisch anspruchsvoll und führt zu einem nicht unerheblichen Gewebstrauma. Der im Folgenden vorzustellende dorsolaterale Zugangsweg verbindet die Vorteile des einfacheren Zugangsweges mit einer minimalen Gewebetraumatisierung. Diese Technik wird mit den konventionellen Zugangswegen zur A. fibularis verglichen.
Patienten und Methoden
Zwischen1984 und 2002 wurden in dem untersuchten Patientenkollektiv 162 Fibularisbypässe angelegt. Bis 1/1991 erfolgte ausschließlich die laterale Freilegung mit Resektion eines Fibulasegments (A, n=11), in der Folgezeit wurde die Technik auf einen medialen Zugangsweg umgestellt (B, n=134). Seit 7/2001 wurde der dorsolaterale Zugangsweg unter Erhalt der Fibula eingeführt (C, n=17).
Ergebnisse
Die Geschlechterverteilung (m.:w.=2:1) und das mediane Alter (67 Jahre, Spanne: 48–87) unterschieden sich nicht zwischen den 3 Gruppen. Die V. saphena magna (VSM) wurde in 54,9% verwendet (A: 45,5% vs. B: 51,5% und C: 88,2%). Die Krankenhausmortalität lag bei 9,1% (A) vs. 8,7% (B) und 0% (C). Distale Wundheilungsstörungen wurden in 0% (A) vs. 21,7% (B) und 0% (C) der Fälle beobachtet. 45% in Gruppe A hatten eine Graftinfektion, 3% in Gruppe B und 0% in C. Die Offenheitsraten betrugen A: 78,9%, B: 66% und C: 84,4% (1 Jahr); die 5-Jahres-Offenheitsrate lag zwischen 28,6% (A) und 36,6% (B). Graftbedingte Unterschiede wurden ebenfalls beobachtet: B (p<0,05): VSM 66,7% vs. Dacron 36% (48 Monate), und 66,7% vs. 27,2% (5 Jahre).
Schlussfolgerungen
Der dorsolaterale Zugang zur A. fibularis stellt eine sichere und effektive Technik im Vergleich zu den herkömmlichen Zugangswegen dar. Er verbindet eine Minimierung des operativen Traumas und eine Verminderung der lokalen Komplikationen. Die 5-Jahres-Offenheitsraten erfordern noch eine detaillierte vergleichende Analyse.
Abstract
Objectives
Surgical access to the peroneal artery (PA) is technically demanding and traumatizing to the patient. The dorsolateral tissue preserving approach simplifies the procedure. We compared this new technique with conventional approaches using a retrospective analysis.
Patients and Methods
A total of 162 PA bypasses were performed between 1984 and January 2002, starting with fibula resection until January 1991 (A, n=11), thereafter medial dissection was practiced until June 2001 (B, n=134). From July 2001 the dorsolateral approach with preservation of the fibula has been used (C, n=17).
Results
Sex ratio (male:female, 2:1) and median age 67 years (range 48–87 years) did not differ between the groups. The greater saphenous vein (GSV) was used in 54.9% (A: 45.5% vs B: 51.5% and C: 88.2%). Hospital mortality was 9.1% (A) vs 8.7% (B) and 0% (C). Distal wound healing was impaired in 0% (A) vs 21.7% (B) and 0% (C), graft infection occurred in 45% (A), 3% (B) and 0% (C). Patency rates were A=78.9%, B=66% and C=84.4% (after 1 year); A=28.6% vs B=36.6% (5 years). Graft-related differences were seen in B (P<0.05): GSV 66.7% vs alloplastic material 36% (48 months), and 66.7% vs 27.2% (5 years).
Conclusions
The dorsolateral approach is safe and effective in comparison with conventional techniques. It is associated with a reduction in operative trauma and achieves longer patency.
