Zusammenfassung
Es wird die Kasuistik einer 47-jährigen Patientin vorgestellt, die unter der Diagnose Thrombophlebitis bei Stammveneninsuffizienz der V. saphena magna mit vermeintlich abgekapseltem Hämatom im Bereich des linken proximalen Unterschenkels operiert wurde. Intraoperativ fand sich aber ein dringend malignomverdächtiger Tumor. Die histologische Aufarbeitung erbrachte ein mesenchymales Chondrosarkom, eine äußerst maligne, aber sehr seltene Tumorentität. Da diese häufig gelenknah unterhalb des Kniegelenks lokalisiert sind, sollten Gefäßchirurgen wie auch Phlebologen sie in ihre differenzialdiagnostischen Überlegungen mit einbeziehen, denn sie werden recht häufig mit dem Leitsymptom der progredienten Anschwellung konfrontiert.
Abstract
This case involves a 47-year-old female patient who was operated following a diagnosis of thrombophlebitis with insufficiency of the internal saphenous vein and putative enclosed bruising in the left lower leg area. During the operation, an urgent, suspected malignant tumor was found. The histological investigation resulted in a diagnosis of mesenchymal chondrosarcoma, an extremely malignant but very rare tumor. Because this type of tumor is often located below the knee, phlebotomists and vascular surgeons should take it into account when making alternative diagnoses, as they are often confronted by progressive swelling as a main symptom.
