Zusammenfassung
Für die Behandlung des zentralen Nahtaneurysmas nach aortaler Gefäßoperation gibt es keinen Königsweg – mit der endovaskulären Aneurysmaausschaltung ist das Therapiespektrum allerdings wertvoll erweitert worden. Die Indikation zur Korrektur orientiert sich an den Kriterien der elektiven Aneurysmachirurgie, für die ein klinikinterner Algorithmus „A.D.A.M.“ (Aneurysmadiagnose – Aneurysmamanagement) entwickelt wurde. Offenes oder endovaskuläres Vorgehen, anatomische oder extraanatomische Korrektur hängen von der Aneurysmamorphologie und den individuellen Gegebenheiten ab (Infektion, Viszeral- und Nierenarterien, Prothesenanschluss). Aus dem eigenen Krankengut wird über 17 Patienten berichtet. Endovaskulär wurden 5 Patienten behandelt (29%), 1 Patient erhielt eine extraanatomische Rekonstruktion. Dieser Patient ist postoperativ an einer nicht gefäßchirurgischen Komplikation (Ulkusblutung) verstorben (Letalität 6%).
Abstract
Treatment of central suture line aneurysms following aortic vascular reconstruction requires treatment tailored to the individual – nevertheless, the addition of the endovascular approach has considerably enhanced the therapeutic spectrum. Whether or not repair of a central suture line aneurysm is indicated depends on the criteria of elective aneurysm surgery; in our clinic, therefore, a special algorithm called A.D.A.M. (aneurysm diagnosis – aneurysm management) has been developed. Whether an open or an endovascular procedure, or an anatomical or an extra-anatomical repair are selected depends on the aneurysm’s morphology and on individual patient-related aspects (graft infection, involvement of visceral and renal arteries, localisation of the central anastomosis). We report on 17 operations performed on our own patients: 4 patients (29%) underwent endovascular procedures, and in 1 case we preferred extra-anatomical reconstruction. This patient died in the postoperative period after bleeding from an ulcer (lethality 6%).
