Zusammenfassung
Die Deckung großer Haut- und Weichteildefekte der unteren Extremität im Rahmen einer peripheren arteriellen Verschlusserkrankung stellt noch immer eine Herausforderung dar. Freiliegende Sehnen nach einem Débridement mit Verlust der Weichteildeckung sind rasch vom Austrocknen mit nachfolgender Nekrose bedroht. Ziel des Wundmanagements muss daher neben der Infektsanierung und Konditionierung des Wundgrundes eine ausreichende Befeuchtung der Sehnen und Faszien sein, um ein Fortschreiten der Nekrosen zu verhindern und eine Granulation zu ermöglichen.
Im vorgestellten Fall eines Patienten mit einem großen zirkulären arteriellen Ulkus des Unterschenkels konnte nach erfolgreicher Revaskularisation durch die Verwendung eines neuen Präparats mit einer Kombination aus einem liposomalen Hydrogel und niedrig konzentriertem PVP-Jod ein Erhalt der Sehnen- und Faszienstrukturen erzielt werden. Die flächige Übergranulation der Sehnen war Grundlage für die anschließende erfolgreiche großflächige Spalthautdeckung. Das verwendete Präparat erzielte eine gute Anfeuchtung des Wundgebiets mit Förderung der Granulation in Kombination mit einer antiinfektiösen Wirkung.
Abstract
It is still a challenge to cover large skin and soft tissue defects in peripheral arterial occlusive disease. After surgical debridement and subsequent loss of soft tissue cover, tendons are quickly threatened by drying and the development of necrosis. Therefore, the goal of wound management, in addition to infection control and conditioning of the wound surface, must be to maintain sufficient moistening of tendons and fascia to prevent the progression of necrosis and to allow granulation.
In the presented case of a patient suffering from a large, circular arterial ulcer of the lower leg, preservation of tendons and fascia could be achieved after successful revascularisation by the use of a new product which combines liposome hydrogel and low-dose povidone-iodine. The surface granulation of tendons was the basis for subsequent successful split skin grafting. Using this preparation, a good moisture level was achieved, thereby promoting tissue granulation as well as exerting an anti-infection effect.
